Benchmark- Part A: Population Health Research and PICOT Statement

The healthcare environment in the modern world is characterized by emerging health problems that increase the need for complex interventions to address them. Health care organizations are forced to explore interventions that would not only promote the efficiency in the use of resources but also optimum health of the population. Achieving these outcomes is highly dependent on the collaborative efforts between different healthcare stakeholders such as the patients, healthcare providers, and management teams of the health organizations. For instance, patients should be willing to adhere to the given regimes for managing their conditions. Healthcare providers should be dedicated to ensure that innovative interventions are used to promote quality and safety in healthcare. Therefore, this paper explores the issue of non-adherence to medication among diabetic African-American patients and proposes a PICOT statement that can guide the adoption of interventions to address it.

Description of Population

African-Americans constitute a significant proportion of the population in America. According to statistics, there are approximately 40 million Africa-Americans in the US. This implies that they constitute about 13% of the population in America. Unlike other ethnicities, people from African-American origin are disproportionately affected by health problems (Iceland, 2017). The disease burden of great focus in this research paper is the poor or lack of adherence to medication among patients of this ethnicity. According to Canedo et al. (2018), African-Americans comprise of an ethnic minority group that have a high burden of diabetes and its prevalence when compared to the whites in the state. There is also the existence of significant medical disparities among this population in terms of access to and utilization of healthcare. This is despite the advances in access to care and medical technologies that aim at reducing the disease burden among them (Canedo et al., 2018). The severity of the disparities can be seen in the report by the CDC where the age-adjusted prevalence of undiagnosed and diagnosed diabetes was high in African-American population when compared to non-Hispanic whites (American Diabetes Association, 2017).

Lack of or poor adherence to diabetes medication is among the factors contributing to the high burden of the disease among African-Americans in the US (Elsous et al., 2017). According to Burnier (2018), adherence refers to the level in which an individual’s behavior such as following a prescribed diet, taking medication, and embracing lifestyle changes aligns with the recommendations from the healthcare provider. Evidence has shown that there is low level of adherence among individuals diagnosed with chronic conditions in developed countries. Accordingly, the level of adherence is approximately 50% in these countries while it is lower in developing nations. It is estimated that 33-69% of hospital admissions in America are attributed to poor adherence to medications. The consequences of the low adherence include worsening of health conditions, increased rates of mortalities, and financial costs incurred by the patients, health care organizations, and the state as a whole (Alqarni et al., 2019). Often, the lack of or poor adherence to treatment among patients with diabetes is attributed to factors such as patient-centered, therapy related, social and economic, disease related, and health care system-related factors (). Therefore, it is important that interventions that aim at raising the level of adherence to medication among African-American patients with diabetes be embraced to improve their health and wellbeing.

In this course, you will be complete a 2-part assignment in which you conduct research about a population of focus, develop a PICOT statement, and write a Literature Review. The PICOT statement and Literature Review you write in this course can be used for your evidence-based practice project in the next course so be sure to select an issue you want to continue working on in your next course.

PICOT (Population/Problem, Intervention, Comparison, Outcome, and Time to achieve the outcome) is a method that helps clarify the qualities needed to create a good question out of a practice issue or problem affecting the population of focus. Additionally, the information derived from a good PICOT makes it easier to perform a literature search in order to find translational research sources that can be used to address the clinical problem.

Use a national, state or local population health care database to research indicators of disparity. Choose a mortality/morbidity indicator to identify a clinical problem or issue that you want to explore pertaining to a population of focus. Use this indicator to begin to formulate a PICOT and conduct research on the population.

Write a 750-1,000-word paper that analyzes your research and focuses on the population you have chosen. Describe the population’s demographics and health concerns, and explain how nursing science, health determinants, and epidemiologic, genomic, and genetic data may impact population health management for the selected population. Provide an overview of a potential solution for solving the health issue related to your population and the intended PICOT statement. Describe how the solution incorporates health policies and goals that support health care equity for the population of focus.

Population

Here you need to fully elaborate upon who the population will be. Include demographics and health concerns. What are their primary characteristics they share? Describe the population’s demographics and health concerns, and explain how nursing science, health determinants, and epidemiologic, genomic, and genetic data may impact population health management for the selected population.

Intervention

Provide an overview of a potential solution for solving the health issue related to your population and the intended PICOT statement. Describe how the solution incorporates health policies and goals that support health care equity for the population of focus. Remember you are not conducting original research but taking already existing research to make a change in practice.

NUR 550 Benchmark – Part B: Literature Review

In Part A, you described the population and quality initiative related to your PICOT (Population/Problem, Intervention, Comparison, Outcome, and Time to achieve the outcome) statement. In this assignment, you will formalize your PICOT and research process.

Use the GCU Library to perform a search for peer-reviewed research articles. Find five peer-reviewed primary source translational research articles.

In a paper of 1,250-1,500 words, synthesize the research into a literature review. The literature review should provide an overview for the reader that illustrates the research related to your particular PICOT. Include the following:

Introduction: Describe the clinical issue or problem you are addressing.

Methods: Describe the criteria you used in choosing your articles

Synthesize the Literature: Part A: Discuss the main components of each article (subjects, methods, key findings) and provide rationale for how this supports your PICOT; Part B: Compare and contrast the articles: Discuss limitations, controversies, and similarities/differences of the studies.

Areas of Further Study: Analyze the evidence presented in your articles to identify what is known, unknown, and requires further study.

You are required to cite five to 10 sources to complete this assignment. Sources must be published within the last 5 years and appropriate for the assignment criteria and nursing content.
