Data Management Plan

A data management plan is a vital part of project management as it ensures that data is timeously collected and the correct sources are identified (Meyke, 2019). Various data types will be collected. Quantitative data will be collected, such as the demographic data on the patients. The data fields include list (both numeric and text values) and date. The data collected will be on the rates of patient falls. Therefore, the frequency of collection will be daily. The data will be collected by the main investigator with the assistance of the unit nurses.   Questionnaires will also be used in collecting data.

Various data sources will also be used in retrieving data. One of them will be the electronic health records. Electronic health records is an electronic form of a patient’s medical history that has been recorded over time. These medical records include a patient’s billing information, the hospital discharge instructions, laboratory test results, immunization status, allergies, medications, health history, ethnicity, gender and age.

As part of the project, the collected data will be analyzed to help in understanding the impacts of the project. A suitable software such as SPSS will be used in analyzing high priority data like counts, averages and percentages. The analyzed data has no meaning without interpretation, hence the results will be interpreted to enhance decision making regarding project outcomes. The services of a statistician will be sought to help in understanding the analyzed data and interpret them within the context.

Various issue can also affect the results, hence it is important to have a plan to mitigate them if they come. One possible issue is bias. The other potential issue that may affect the results is data incompleteness. The organization also needs to protect the data. Some of the patient data to be collected and private and confidential, hence the data will protected through various strategies such as encryption and restricting access to the data using strong passwords (Meyke, 2019).

The graduate collaborates with internal and external stakeholders to identify key success indicators for evaluating the effectiveness of a healthcare improvement project.

The graduate determines the most effective technology and data sources for capturing data that will measure the key performance indicators (KPIs) for a health improvement project (HIP).

7073.05.3 : Data Management

The graduate creates a data collection plan that includes the data fields and types, frequency of collection, data collector, and the data sources for retrieving and entering data.

7073.05.4 : Evaluation Plan

The graduate initiates the process of collaboratively developing a healthcare improvement evaluation plan that addresses stakeholder questions and high-priority areas to measure progress over time and describes contextual issues that may impact the results.

After your preceptor or SME completes this portion, the signed document will be emailed to you to sign as well.

 

Finally, you must upload the “CPE Record” and the required deliverables from that record to your e-portfolio. You should complete this task before submitting JIM1 Task 1.

 

REQUIREMENTS
Your submission must be your original work. No more than a combined total of 30% of the submission and no more than a 10% match to any one individual source can be directly quoted or closely paraphrased from sources, even if cited correctly. The originality report that is provided when you submit your task can be used as a guide.

You must use the rubric to direct the creation of your submission because it provides detailed criteria that will be used to evaluate your work. Each requirement below may be evaluated by more than one rubric aspect. The rubric aspect titles may contain hyperlinks to relevant portions of the course.

Tasks may not be submitted as cloud links, such as links to Google Docs, Google Slides, OneDrive, etc., unless specified in the task requirements. All other submissions must be file types that are uploaded and submitted as attachments (e.g., .docx, .pdf, .ppt).

A. Upload your completed and signed “CPE Record” for evaluation as a separate document. The “CPE Record” should include the following:

• each completed requirement with hours that are initialed and dated by your supervising preceptor or SME

• electronic signatures verifying that all task hours have been completed

B. Upload each of the following required deliverables to your e-portfolio and provide a link to your e-portfolio:

• CPE schedule table with tasks and timeline

• sample project report

• three screenshots to document your Phase 1 GoReact video reflection, including an image of your reflection video and an image for each of your peer responses

• written summary of your Phase 1 video reflection

• data management plan

• three screenshots to document your Phase 2 GoReact video reflection, including an image of your reflection video and an image for each of your peer responses

• written summary of your Phase 2 video reflection
