Health care providers should play an advanced role in enhancing patient outcomes by proposing evidence-based interventions. They should be centrally involved in change implementation to ensure high-quality care and optimal patient safety. Mostly, practice change is achieved through evidence-based practice (EBP) projects. Such projects examine a clinical problem in detail and recommend changes based on scientific evidence. Successful change implementation requires health care providers to apply a model or framework for change. The purpose of this paper is to discuss the selected model and its relevance to my project, the model’s stages, and how it can be applied in project implementation.

The Model and Relevance

I selected the Johns Hopkins Nursing Evidence-Based Practice model to guide the change process. Duff et al. (2020) explained that EBP change models provide a structured approach to change implementation in elaborate steps. The same case applies to the Johns Hopkins model in my process. It provides a process to follow to ensure that change is adopted with a specific purpose. The model also promotes a culture of care based on evidence by allowing rapid application of current, relevant research and best practices to improve patient outcomes (Dang et al., 2021). Applying the model implies that all recommendations are evidence-based and the project will promote positive change as intended.

Stages of the Johns Hopkins Model

The Johns Hopkins model facilitates that through the PET process. This process involves three basic stages: practice question, evidence, and translation (Upstate Medical University, 2021). After identifying the clinical problem, health care providers formulate a practice question to guide evidence search. The search for evidence is an intensive process. Based on the question, health care providers conduct an in-depth search of current, scientific, and relevant evidence followed by an appraisal of its level and quality. Next, the evidence is summarized, and recommendations for change are developed based on the synthesized evidence (Upstate Medical University, 2021). The last stage involves actualizing the recommendations through an action plan. The organization’s management’s role is critical in this stage to support change and provide resources to implement the action plan.

Applying the Stages in My Proposed Implementation

PICOT question: In the perinatal women (P), will the treatment of a hypertension crisis with the recommended medication within 60 min (I), compared to current standardized practices (C), improve maternal outcomes/prevent complications (O) within 6 months (T)?

The PICOT question will guide the search for evidence. The focus will be on high-level evidence evaluating the efficacy of hypertension crisis treatment with the recommended medication within 60 minutes. Such evidence includes Wang et al.’s (2018) study on the management of morning hypertension and a review by Carey et al. (2018) on the complications that can be prevented by prompt hypertension treatment. Next, I will appraise the evidence and summarize the findings depending on the relevance to the clinical question. Best practices based on evidence are then implemented through an action plan. However, if a nurse is not actualizing the recommendations, they can provide a summarized report of findings to guide positive practice change, such as preventing complications in perinatal women.

Conclusion

Practice change should be a systematic process. EBP change models provide basic outlines for change facilitation to achieve a specific outcome. As explained, the Johns Hopkins nursing model facilitates change in three basic steps. It will help guide my project since current scientific evidence is required to assess the efficacy of the proposed hypertension treatment. The evidence will be appraised, and the findings relevant to the clinical question summarized as best practices for improving outcomes.
