Effective communication among providers will ensure that they develop integrated care plans to offer the best quality care for patients in different conditions.

Technologies Needed for the Project

The implementation of the proposed project on telehealth in the facility will require critical technologies to be successful and capture the aspirations of all stakeholders; right from nurses to patients as the primary beneficiaries. The project will require wearable monitored smartphones, portable devices like wrists and alert watches, laptops, and alarm systems integrated in critical areas of care provision like the nurse station. The project will also require a standard server to integrate this data and system with the rest of the organization’s system (McGonigle & Mastrian, 2018). The organization’s IT department in collaboration with the nursing department and informaticist nurse will select the most appropriate vendor for the technologies. Having one vendor or organization supply these technologies will be desirable and will enhance increased synchronization. The informaticist nurse will also conduct training on the technology to ensure that all stakeholders, especially end user, area familiar with its working.

The Project Team & Roles

The team for this project will play a critical role in ensuring full implementation and adoption of the initiative to enhance patient outcomes and efficiency in care provision. Imperatively, the project team will comprise of director of nursing, nurse manager, project manager, informatics nurse, representative of the physicians and therapists, and IT departmental manager as well as a representative from the organizational management and leadership. Every nursing staff will be onboard once the project is rolled out after testing and dry run. These include the check-in area, admissions and discharge staff. The role of nurses, physicians and other healthcare providers would be to provide information about different facets of the project and how they fit their working to improve outlined outcomes and efficiencies. The nurse informaticist will also carry out orientation for any department and its staff to familiarize with the new system. The informaticist will also engage the IT team for any changes to system and even upgrade to improve the quality of service provided.

The implementation of this project will be critical in improving overall care provision for patients and ensuring that they get quality care outcomes. The proposed project leverages existing telehealth technologies and the benefits that they offer to enhance quality outcomes among patients and efficiencies for nurses. The incorporation of nurse informaticist will be ensure that the system aligns with organizational needs that include increased efficiencies.
