Competing needs in health care system affect how organizations develop policies and interventions to remain sustainable and offer quality patient care outcomes. Nursing shortage is a serious national health care stressor and competing needs exacerbate the issue due to organizations prioritizing certain aspects at the expense of others (Spurlock Jr, 2020). Nursing shortage occurs due to different factors in retaining and hiring of nurses and thus attaining a balance between resources like tight budgets, enhancing patients’ health and wellbeing of nurses is challenging for many health organizations and their managers.

The two competing needs related to nursing shortage include equitable allocation of resources and reducing the overall cost containment of operations to attain expected profits or return on investment. Nurses encounter increased workload daily due to a host of factors like high patient-to-nurse ratio, increased levels of acuity and patient assignments that may be out of their comfort zones. Nurses are overwhelmed and have limited time for themselves. Conversely, healthcare entities and their management are keen on cutting costs to remain sustainable leading to limited human resources to help nurses reduce their workload (Thomas et al., 2019). The implication is a nursing shortage as nurses leave due to fatigue and burnout. Nurses also experience increased levels of stress because of staffing requirements, understaffing and due to limited administrative support.
Relevant Policy & Practices to Address the Issue
Optimal allocation of resources and efforts to attain relevant nursing staff levels are essential consideration in developing policies and practices to address nursing shortage. In this case, the best policy is to increase retention of nurses, from currently 60% to 90% within two months to address the problem of nursing shortage. Retention policy that focuses on improving practices like orientation, offering training to new nurses, having incentives in financial form to improve personal growth, and sufficient support from the organizational leadership and management will help address the current issue. The organization should invest in these areas to motivate nurses so that they do not leave the facility. Again, such a policy would ensure that nurses have sufficient time for other activities like research and collaboration with other stakeholders to improve patient care delivery.
Ethical Considerations: Strengths & Challenges
Organizations, especially in healthcare, establish policies to protect nursing staff and ensure patient safety while addressing the arising competing needs. According to Milliken (2018), nurses should have ethical awareness to offer safe care to patient in the best way. Ethical awareness is essential for nursing staff and nurse leaders to address any issues while developing policies to tackle nursing shortage. For instance, the above policy may provoke mixed reactions from nurses with some feeling that it does little to protect them from exposure to more workload.
Ethical considerations in this case would entail involving nurses to understand the importance of the policy and allowing them to voice concerns about the benefits and challenges of the proposed intervention (Kelly et al., 2018). The proposed policy will allow nurses to enhance their skills and knowledge, improve their ability to handle patients, and reduce adverse events. However, it requires increased support from the organizational leaders and managers. More fundamentally, it requires resources which remain a challenge for many organizations.
Recommended Practice Change
Collaborative approach to the issue and increased leveraging of technology will help the organization attain a balance among the competing needs of resources, employees, and patients. Collaboration will ensure that all stakeholders work based on a common purpose while leveraging the advantages provided by technologies like telehealth and telemedicine. These technologies will ensure that patients can access care at remote locations and reduce the need to make physical visits to hospitals. The current policies are not well designed to consider the ethical aspects of care provision for patients and nurses. These policies do not address ethical harm associated with increased workload for nurses and patient safety.
