Assignment 1: Lab Assignment: Ethical Concerns

The case study assigned demonstrates a 47-year-old male patient admitted to the emergency room with a primary diagnosis of ulcerative colitis and symptoms of extreme abdominal pain. The patient has failed to take his medication for the past few months due to a lack of medical insurance. Further assessment of the patient’s condition will involve finding the actual location of the abdominal pain and associated symptoms such as blood in stool, fever, and diarrhea (Kucharzik et al., 2020). The patient’s frequency of bowel motion per day, in addition to bleeding rectum, will also be considered for grading purposes, as either mild, moderate, or severe. To confirm the patient’s primary diagnosis, a mucosal biopsy for histopathology and endoscopy will be ordered (Nakov, 2019). Additional diagnostic tests needed to assess for associated complications include a complete blood count for anemia and thrombocytosis; a comprehensive metabolic panel for hypokalaemia, hypoalbuminemia, elevated alkaline phosphate, and hypomagnesemia; inflammatory markers; and stool assay.

For better treatment outcomes among patients with ulcerative colitis, evidence demonstrates the need of being compliant with medication therapy, before considering additional non-pharmacological approaches such as dietary interventions (Kucharzik et al., 2020). As an APRN, patients with no medical insurance require equal attention to the observation of specific ethical standards such as beneficence, justice, nonmaleficence, and respect for the person. As such, it will be necessary to consider the patient’s financial burden and prescribe low-cost generic medications that the patients can afford (Fernandez-Lazaro et al., 2019). Consequently, the APRN can also connect the patient with state-based assistance plans, pharmaceutical company-based assistance plans, and pharmacists who provide 30-day supplies of several prescribed medications (Kvarnström et al., 2021). The above strategies will help the patient acquire the prescribed medication at a reduced cost hence promoting his compliance and recovery.
