Diabetes mellitus (DM) remains a critical healthcare challenge that requires providers to use effective change models to enhance self-management care and behaviors. Patients with diabetes mellitus require interventions based on nursing education to understand the significance of self-care. The need for change in diabetes self-management requires use of certain change models that include the Iowa model whose focus is health promotion (Albarqouni et al., 2018). The purpose of this paper is to demonstrate the use of Iowa model in health promotion for effective self-care management for individuals with diabetes mellitus.


Selected Model or Framework and its Relevance to the Project

The Iowa model is an effective EBP framework, which focuses on the whole healthcare system consisting of patients, healthcare providers, materials and resources. The framework assists and offers guidelines to the implementation of decisions in self-care management for diabetes mellitus patients based on the use of available research evidence. The model is suitable for this project since it is detailed and uses a pragmatic problem-solving approach in EBP implementation and takes an organizational system approach to attain the desired outcomes to the current clinical issue (Klingbeil & Gibson, 2018). The model enables stakeholders to implement the EBP process in questioning and critically assessing the current nursing practice and if they can improve care using research evidence.

Stages of the Iowa Model & Application in the EBP Project

The Iowa EBP model comprises of seven stages, which focus on the need for nurses to follow hypoglycemic protocols to enhance diabetes control and treatment of patients taking insulin. The first part of the model is the identification of the problem that needs change. The issue here entails the need to use effective protocols in diabetes management, especially taking insulin to control the condition. The second stage of the model entails showing that there is a priority for the provider or facility to implement change in practice or intervention (Klingbeil & Gibson, 2018). In this instance, there is an enhanced level of priority for understanding and implementing effective protocols to attain hypoglycemic control and treatment of patients. Evidence shows that having effective hypoglycemic protocols allows patients to control adverse diabetic events and enhances the quality of care as it leads to better self-care management (Sorour eet al., 2020). The third step in the model is having teams to design, implement, integrate and evaluate EBP changes. The team should comprise of interested stakeholders with the purpose of appraising the intervention, synthesizing the available literature and presenting findings for implementation of the recommendations. In this case, the team will consist of nurses, physicians, pharmacists and other critical stakeholders like caregivers to design the program and its implementation.

The fourth stage entails gathering and analyzing research associated with the desired change by evaluating the level and merit of the evidence. The model implores one to create a PICOT question and statement to conduct evidence search for relevant studies. The fifth stage is the review and analysis level aimed at getting literature and ensuring that the evidence supports the EBP intervention (Melnyk & Fineout-Overholt, 2019). For instance, the existing evidence shows that hypoglycemic protocols enable patients to control their diabetes through effective use of insulin. The sixth stage entails appraisal of the PICOT practice change based on the degree of success in overcoming the identified aspect in practice setting. The last step is implementing the changes realized through the pilot practice and evaluating outcomes, especially if it is cost-effective and sustainable or if it requires changes.


The Iowa model shows that planning and action process that include relevant research evidence are essential when implementing EBP interventions like the attainment of effective controls of blood sugar levels among diabetic patients with diabetes mellitus. The model makes sense as the most effective because it is straight forward to embrace and appropriate for the said intervention. The implication is that the application of the model will enhance the participation of patients and their caregivers in controlling blood sugar levels among individuals with diabetes mellitus
