Addressing population health problems requires carefully identifying ideal interventions to suit a particular problem and intervention. Cogburn (2019) notes that addressing population health issues helps reduce health disparities and enhance equity. This discussion evaluates a strategy that can be used to address a population health practice problem to reduce health disparities. A Healthy People 2030 goal that can be applied to the selected health issue and population will be identified, propose an evidence-based intervention to address the goal and determine whether the intervention is effective, efficient and efficacious.

The selected practice problem is diabetes; the population is African Americans living in Jackson County, Mississippi. Diabetes is a major health issue in this population, leading to increased healthcare expenses, reduced productivity, and poor life quality. Therefore, it is crucial to identify evidence-based interventions that can be used to address the health issue in the population. The healthy people 2030 goal for diabetes among African Americans is “Reduce the burden of diabetes and improve quality of life for all people who have or are at risk for diabetes” (Healthy People 2030, n.d.). The goal aims at applying evidence-based interventions and strategies to prevent and manage diabetes across health populations, thus reducing new diabetes cases and enhancing proper management for already diagnosed individuals.
The evidence-based intervention that would help address the diabetes health issue in line with the Healthy People 2030 goal is lifestyle modification. According to Lynch et al. (2019), lifestyle intervention aims at modifying people’s lives to prevent and manage diabetes, considering that the condition is mainly influenced by lifestyle. The intervention entails nutrition, physical exercise, diabetes management, smoking cessation, and lowering alcohol consumption, supporting diabetes prevention and management.
It is essential to determine an intervention’s effectiveness, efficiency, and efficacy. Grimani et al. (2019) note that the effectiveness of an intervention is its ability to have meaningful effects on patients. In this case, I would determine if the lifestyle change intervention is effective by assessing the participants’ changes and how they contribute to diabetes prevention and management. I would also determine its efficiency by assessing whether the costs used in the intervention are low and economical. Additionally, I would determine the efficacy of the intervention if it is ideal to lead to reduced diabetes cases and proper diabetes management.
The lifestyle modification intervention aligns with the healthy people 2030 goal on diabetes and can be used to address the issue in the selected population. The intervention’s efficacy, efficiency and effectiveness will be determined to ensure that it is the ideal intervention to lead to the needed solution.
