The Expected Outcomes

            The proposed project seeks to determine whether including family as a support system in educating non-English speaking patients with hypertension improves blood pressure outcomes. The evidence-based proposal seeks to improve self-care among non-English speaking patients with hypertension. The expected outcome from this project includes reduced readmission rates, reduced cases of hypertension mortality, reduced hospital visits due to complexities of blood pressure, and increased discharge summaries for hypertensive patients. The increased readmission in a hospital causes more harm to patients in terms of additional cost and lost trust in the quality of healthcare delivery (Eriksen & Frandsen, 2018).             The study done by Charach et al. (2022) found that complications of hypertension can be detrimental to patients, especially when they cannot seek faster medical attention. Such patients present a blood pressure higher than 160 systolic and 110 diastolic. These cases could increase the risk of heart failure, brain damage and heart attack. The life of these patients depends on how fast they can receive medical attention, while in some severe cases it can lead to the patient’s sudden demise. Therefore, it is important to educate patients on ways of improving blood pressure at home to limit instances of severe hypertension.

Data Collection Tool and Data Collection Method

            The data to be collected in this project will be from electronic health records (EHRs). Each of the considered patients has their data within the EHR system at the healthcare institution. Therefore, the tool will be a significant source of data collection for this study. The data from this system will show if the proposed intervention is effective and can meet the desired patient outcome concerning the management of blood pressure among non-English speaking patients with hypertension. Electronic health records capture accurate patient information because they are used in the daily monitoring of patients at the institution (Brownson et al., 2018). This makes the tool significant in this study.

            Chart audits will also be used in this project. Chart audits done internally are used to measure activities that aid in supporting an organization to achieve its main objectives, the evaluation of care quality, and the assessment of staff performance. This tool will be designed to have a series of questions that seeks to clarify whether the family support system was included in the patient education program. Besides, the tool will include the initiation readings of the blood pressure before and after the intervention. It also includes if the nurse in charge was notified of the readings. The project drivers will work closely with the healthcare professionals to discuss the information captured from the EHRs (Eriksen & Frandsen, 2018). The outcome will be measured in percentages, and the data will involve determining consistency in the data collected.

Statistical Tests

            This project will use an interval test. This statistical test is selected because it can determine whether the outcomes on blood pressure vary within the two interventions. The trend recorded on patient outcomes before the intervention will be compared to the trend noted after (Terwee et al., 2018). This comparison would provide the most effective conclusion that will either support or decline the proposed project. The Interval test is suitable because it can compare two measurable outcomes. Furthermore, the tool is compatible with numerous statistical tests, especially in the analysis of EHR data used in supporting quantitative data to be used in a project.  

Remediation Strategies

            The drivers of this project operate on the premise that the expected outcome may be positive or fail to meet the expected outcome. In case the project fails to meet the expected outcome, the research methodology would be reexamined for any bias and revised to align with the expected outcome. If the unexpected results persist after this process, an intense comparative analysis with findings from other studies will be done to decide whether to redo the intervention. The nature of the proposed change must be accorded with sufficient data to limit controversies on the practice (Eriksen & Frandsen, 2018). Therefore, it would imply that the project drivers and the stakeholders would commence the project from scratch to review the implementation steps. They would also think of other options that failed the project. These options may include speaking with other non-clinical staff to identify other barriers and conducting a review on EHRs twice a month instead of once a month.

Plan to Maintain and Extend the Proposed Solution

            Maintaining the proposal after implementation will be achieved by engaging nurse leaders and other frontline workers because they are the main drivers of this project. The healthcare providers must receive satisfactory training on the implementation and sustenance of the proposed solution when the data shows that educating non-English speaking hypertensive patients in the presence of their families improves blood pressure outcomes (Albarqouni et al., 2018). Social media, posters, and online presentations with recommendation medication will be used to remind healthcare professionals to adopt this proposed intervention constantly.

Conclusion

            The choice of data collection tools and methods will be significant in measuring the outcomes of the proposed change. The proposed evidence-based practice is based on data that supports its use in healthcare practice. The findings from this research will be used to examine the proposed change and make informed decisions based on data. It is important to develop these steps to evaluate the plan early enough to increase the chances of implementing the project in healthcare.
