Evidence-Based Practice Project Proposal: Identification of Nursing Practice Problem
Part 1: Falls are a major health problem globally with sequelae ranging from minor abrasions and bruises to more complex consequences like head injuries, fractures lacerations, and death. Mortality rates among age-adjusted falls have increased over the past few years. According to the Centre for Disease Control and Prevention (CDC), every year, approximately one million patients fall in hospitals within the United States accounting for roughly 85% of all hospital-acquired conditions(Dykes et al., 2020).Incidences of hospital fall and injury vary based on the unit characteristic, with increased rates among medical-surgical patients as compared to intensive care patients. In the United States, medical-surgical units report approximately 3.67 to 6.26 falls per 1000 patient days (PDs). Several evidence-based strategies have been utilized in fall prevention. However, the Center for Medicare and Medicaid (CMS) identifies falls as a “never event,” leading to a substantial amount of pressure on hospital administrators hence no falls goals in most hospitals. Advanced nurse practitioners bear the responsibility of providing direct patient care and promoting fall prevention interventions. This project will thus focus on evaluating the benefits associated with the use of the Tailoring Intervention for Patient Safety (TIPS) toolkit to reduce the incidences of falls on a medical-surgical unit.
