Interventions

  1. Make postpartum care an ongoing process rather than a single contact checkup

Create constant monitoring and followup programs for pregnant women from the prenatal period to one year months postpartum. In most cases, postpartum care occurs as a single contact when women come for checkup. Moreover, there is barely any checkup for young mothers three months post-delivery. Yet, adequate support for young mothers requires follow-up and monitoring for at least six months. In this intervention, the clinic will create a program that ensures that checkup from prenatal to postnatal is an ongoing care process rather than once in a while. Additionally, the monitoring for young mothers would occur up to one-year postpartum. This will help keep track of the health of the mother and baby and provide support where necessary to enhance health outcomes. Outcomes will be measured by increased access to prenatal and postnatal services by pregnant women and mothers. Other

Education and health promotion programs for pregnant women and postpartum mothers on self-care and healthy eating support their health and wellbeing (Herval et al., 2019). The outcome will be measured by the reduced number of women reporting anxiety and other stress-related issues such as postpartum depression.

Improved patient outcomes

The interventions are based on preventive measures to mitigate the health risks that affect pregnant women and infants. For example, education on proper nutrition will ensure that women give birth to healthy babies, reducing the chances of hospitalization and associated costs (Pallangyo, 2018). For instance, when a woman follows a healthy nutrition regime during pregnancy, they are likely to give birth to a healthy baby that will not be susceptible to health problems such as premature birth, low birth weight, or deformity. Similarly, the initiatives will mitigate the risk of postpartum depression prevalent among new mothers and eliminate unnecessary costs. Moreover, prolonged monitoring will reduce cases of neonatal, sudden infant deaths, and maternal mortality. Similarly, psychosocial support has several beneficial outcomes in prenatal and postnatal women, including low rates of depression. A study by Moseholm et al. (2022) confirms that women who receive psychosocial support are likely to record low rates of depression.

NP ratings

NP ratings are measured by the improved health and wellbeing of populations and reduced cost of care. The interventions focus on the prenatal and postnatal population, which will lead to improved quality of life, reduced cases of illness, and overall wellbeing. For example, the likely measurements will be low premature births and infant deaths. Additionally, monitoring will ensure a good experience for women, encouraging them to adhere to exclusive breastfeeding for six months, which is essential for the baby’s proper health (Moseholm et al.
