Despite tremendous scientific and technological improvement, medication errors are still common in healthcare settings, necessitating thorough investigation and understanding of their underlying causes. Exploring various aspects, such as breakdowns in communication, system deficiencies, organization factors, and human factors that contribute to the high number of medication errors, can perhaps help address this issue. This paper aims to shed light on the different factors and causes of medication errors in healthcare settings to help bridge the gap between practical application and theoretical knowledge.
