The California Endowment’s Healthy Eating, Active Communities (HEAC) Program

 

Description
  • The California Endowment established the HEAC program to lower disparities in obesity and diabetes by improving food and physical fitness environments for school-age children.
  • The program was launched at six sites across California.
  • HEAC was developed to address five key environments or sectors that impact children’s nutrition and physical activity behavior: Schools, After-School Programs, Neighborhoods, Health Care, and Marketing and Advertising (Samuels et al., 2010).
  • The program was designed to prevent childhood obesity through collaborative action that improves the local environment to make healthy living possible.

 

 

How was the success of the program or policy measured?

 

 

The program’s success was measured by assessing:

i.        Changes in foods and beverages sold at schools and in neighborhoods in HEAC sites.

ii.        Changes in school and after-school physical activity programming and equipment.

iii.        Individual-level changes in children’s attitudes and behaviors related to food and physical activity (Samuels et al., 2010).

iv.        HEAC-related awareness and engagement on the part of community members, stakeholders, and policymakers.

 

 

 

 

How many people were reached by the program or policy selected? How much of an impact was realized with the program or policy selected?

 

  • The California Endowment chose six low-income communities as HEAC program sites with populations ranging from 15 000 to 90 000.
  • Eleven school districts, ranging from rural to urban, introduced 885,000 students to HEAC interventions across the 6 HEAC sites (Samuels et al., 2010).
  • Since the HEAC program’s inception, substantial improvements have been noted in HEAC sites in the healthfulness of school environments. For instance, all the HEAC school districts have met the physical education standards, competitive food standards, and strong restrictions on advertising and marketing foods and beverages on school campuses (Samuels et al., 2010).

The HEAC program was evaluated at the midpoint, that is, two years after inception.
What data was used to conduct the program or policy evaluation?

 

 

Data used in evaluating the HEAC program was based on the children’s environment:

  • Schools:  Resources, equipment, and physical activity opportunities available in schools.
  • After-school: Beverage and a la carte food offerings and sales in after-school settings.
  • Health care: Beverage and food sales and physical activity opportunities in health care facilities and public health departments and how health care providers address obesity prevention and promote good nutrition and physical activity in their clinical practice (Samuels et al., 2010).
  • Neighborhood: The physical activity environment and resources in neighborhoods.

 

 

 

 

What specific information on unintended consequences were identified?

 

  • The current economic conditions have decreased the resources available to public and private agencies in the HEAC communities.
  • Schools taking part in the HEAC program have made severe budget cuts down to programs, such as physical education, that had been reinforced with the HEAC dollar (Samuels et al., 2010).
What stakeholders were identified in the evaluation of the program or policy? Who would benefit most from the results and reporting of the program or policy evaluation? Be specific and provide examples.

 

 

  • Stakeholders identified in the program’s evaluation include school-going children, public health departments, schools, community organizations, and the local government.
  • Children would benefit the most from the results and reporting of the HEAC program since the positive impact would influence other states to adopt a similar program to help lower the childhood obesity rate, which is a growing public health concern.
  • For example, reporting the program’s results would influence local government, public health departments, and school districts to collaborate in developing a similar program. This would promote healthier dietary habits and increase physical activity levels for children to lower their risk of developing obesity (Korn et al., 2018).

 

 

 

 

 

Did the program or policy meet the original intent and objectives? Why or why not?

 

 

  • The HEAC program successfully met its original aim since the children’s environments changed to promote healthier lifestyles across various domains in all the five key childhood environments for all six HEAC communities.
  • Children in the HEAC communities are engaging in more healthy behaviors than before the program’s implementation (Samuels et al., 2010).
  • HEAC demonstrated that it is possible to make major improvements community-wide in the food and physical activity environments that children reside in.

 

 

 

Would you recommend implementing this program or policy in your place of work? Why or why not?

 

 

  • I would strongly recommend implementing a program similar to HEACS in my organization to curb the increasing rates of childhood obesity.
  • It would promote healthier lifestyles for the children we serve and thus lower the complications associated with obesity that are often diagnosed in our facility, such as diabetes mellitus.

 

 

Identify at least two ways that you, as a nurse advocate, could become involved in evaluating a program or policy after one year of implementation.

 

 

As a nurse advocate, I would become involved in program evaluation by:

  • Taking part in determining the evaluation priorities. I would take part in making decisions on which interventions and outcomes should be evaluated (Adams & Neville, 2020).
  • Disseminating the program evaluation to interdisciplinary health care audiences interested in the health problem of focus or the approaches implemented to address the problem.

 

 

 

 

 

 

 

 

 

 

 

 

General Notes/Comments  

  • Efforts at improving children’s fitness and academic achievement complement rather than competing with each other.
  • Providing high-quality physical education in schools promotes academic improvements. This demonstrates the value of high-quality after-school activities within a high-quality instructional program.

 

 

Conclusion

Healthy Eating, Active Communities (HEAC) Program was established in California to lower disparities in obesity and diabetes by improving nutritional and physical fitness environments for school-going children. The selected sites for the program successfully changed children’s food and physical activity environments, promoting a healthy lifestyle for low-income children and their families. I would recommend a similar program in my organization to promote healthier lifestyles in children and lower the risk of developing obesity and associated comorbidities.
