Leading Organizations for Quality Improvement Initiatives Assignment

The organization is a famous healthcare center in Baltimore, Maryland. The organization is known for the quality of healthcare services and its relationship with the community. The meeting held with the organization included nurse leaders, surgeons, critical care nurses, and administrative managers. Each person included in the meeting had a role to play as far as improvement initiatives were concerned. The nurse managers explained some of the challenges they face in assigning roles to nurses in the ICU and some of the measures they feel the clinic should be taking to meet the desired outcome (Cho & Steege, 2021). The surgeons working in the ICU were present in the meeting, and they also talked about how the issue of staff shortage in the ICU affects their role negatively. The critical nurse was also included in the meeting to understand the perspective of the ICU nurses and issues affecting the quality of their work output. The purpose of this assignment is to discuss the meeting summary I had with the organization.

I approached the organization by writing an official letter to the management. The secretary received the letter and later delivered to the manager, who agreed to my concerns and to have the meeting with their officials. The manager granted my wish because I was a DNP student who was undertaking my project at the clinic. Again, the clinic manager has always seen the impact of DNP students in leading improvement at the facility, and this opportunity was seen as one that would improve the services at the clinic (Knupp et al., 2018). These students carry out new projects that have proved significant in improving healthcare.

I selected the organization because of its size and the more healthcare activities it can offer to the patients. The information I could receive from the arrangement would be important in gaining more insights about the organization. Again, it would be important to identify the gaps that need additional improvements at the clinic. I was sure that the management would be ready to meet with me, especially regarding issues related to quality improvement at the clinic.

The organization has a key person overseeing students undertaking the institution’s projects. The management assigns a professional nurse to all the DNP students at the clinic. The assigned nurses train the student on ways of accessing important data they may be indeed of while at the facility. Besides, they offer background rules at the clinic, which they must follow in adhering to the set regulations.

The DNP Project’s Description

Explaining my DNP project to Walden University started by pointing out facts on the gaps I identified at the clinic. The selected problem for this discussion was long shifts among the nurses working in intensive care units at the organization. I further explained that there are a few reasons why long nursing shifts can be a problem in ICU in any healthcare organization (Cho & Steege, 2021). For one, tired nurses may not be as alert as they need to be, leading to mistakes and potentially dangerous situations. In expounding on my DNP project, I illustrated that nurses who work long shifts might not have enough time to eat or rest, which can also lead to problems (Joshua et al., 2021). Again, when nurses are overworked and exhausted, it can be difficult to form strong relationships with their patients and provide the care they need.

The participants questioned the extent of my identified problem in the clinic and how I would access the data that could be used to prove the problem in my DNP project. In response to this, I explained that I had been assigned a nurse professional in my identified organization, and they have agreed to provide data that supports my DNP project. These questions were important as they allowed me to prove my project’s significance.

The reduction in quality performance at the ICU has been an issue that the organization has wanted to solve for many years. The complaint about the decline in the desired outcome resulted in an employee survey that revealed a problem with employee staffing. Nurse fatigue is a significant problem in the ICU and has been identified as contributing to medical errors. Studies carried out by various scholars also contend that nurse fatigue is a major problem in the healthcare setting and a major contributor to medication errors in critical care units (Joshua et al., 2021). A study by the National Institute for Occupational Safety and Health (NIOSH) found that nurses who worked long shifts (12 hours or more) had an increased risk of suffering from fatigue-related injuries. These sources were significant in explaining the gaps and addressing the topic of my DNP project.

The approach proposed to solve this problem was for the organization to reduce the working hours for nurses at the unit. This quality improvement initiative aligns with the American Nurses Association guidelines for shift lengths. The association recommends that nurses should not have more than 12 hours for a single shift and no more than 36 hours over three days (Alshahrani, 2020). Nurses should also be given time off between shifts to allow for adequate rest. Fatigue can negatively impact the nurse’s performance and lead to mistakes that may harm patients. Again, the organization should make plans to hire more critical unit nurses who will aid in solving the staff shortage at the clinic. Moreover, capacity building through training could also save the resources which could have been used in hiring other new nurses. Training existing nurses to work in the critical care unit would reduce the entire expense of reducing the nurse shortage.

The organization officials were concerned about how effective the implementation of my proposal on the issue would take place at the clinic. They asked about the proposed change’s financial impact on the clinic. The nurse manager asked which choice would best fit the organization (Gao et al., 2020). I replied that training the existing nurses to work in the ICU would be cost-effective to the clinic as the nurses are already employees of the institution. These proposed solutions would lead to positive cultural change by providing the organization with another chance to hire and select nurses from diverse ethical backgrounds.

Project Team

Various stakeholders will be included in the project to address the identified gaps. The stakeholders will include nurses at the ICU, finance officers, nurse managers, and overall nurse managers at the clinic. Nurses at the ICU will  actively be involved in the implementation process of the proposed project by availing themselves of additional training. As a result, their role in the implementation process is essential. They will also reduce the risk of resistance from other nurses (Cho & Steege, 2021). Nurse managers at the clinic will be involved in the entire coordination process. They will provide feedback on the successes, challenges, and opportunities to explore for the success of this project. The financial manager will determine the financial implications of the project and perform a cost-benefit analysis that will show whether the project is significant or not.

Next Steps

The successful agreement with the stakeholders on the proposed project will require human and financial resources to achieve its desired outcome. The human resources will include nurse managers, healthcare workers, and trainers for the trainees who will be equipping the nurses with the new roles of working at the ICU (Cho & Steege, 2021). The main question in this section was the rough budget that the organization needs to be projecting for the completion of the project. I projected that the clinic needs about $84,000 to undertake this process. They nodded in agreement that the budget was good for the company and discussed when to commence the projected change process.

Conclusion

The meeting has effectively allowed me to explain my observation at the clinic and a solution that needs to accompany the project. I would ensure that the clinic adopts my proposed change to enhance the quality of healthcare services. I would need to have a view of the metric performance at the ICU for the last year. This data would allow me to understand the data and ensure its successful implementation in the process.
