Based on your accepted Milestone one assignment, you will use the information to identify three teaching areas to improve patient outcomes. You will also determine three ways to evaluate the success of the patients use of the mHealth hap. Be sure to use the template worksheet provided for you for week four, Milestone two. Here is a copy of the course project milestone two template that you will use in this assignment. Please be brief and use bullet points. Bullet points are not full sentences and should be bullet point for each section. In Milestone two. We are preparing this portion of the project for the upcoming assignment in Milestone three, which will be a PowerPoint or poster presentation. Remember your milestones should look differently than mine. You will want to use your chosen topic and examples for your own course project. This next section includes the identified teaching areas related to the mHealth app. These three teaching areas are meant to improve patient outcomes. Define Approved Patient Scenario
The scenario involves poorly controlled diabetes. Frank Jones presents himself at the community health center with type 2 diabetes, whose control and management have become a significant family problem. Frank has struggled to keep glucose levels within the desired health range in the past six months. Other concerns include carb intake control and timely data sharing with his doctor. As a result, he wants advice and medical assistance on overcoming these problems and achieving optimal health. Based on his concerns and questions, Frank shows readiness to learn about the effective management of type 2 diabetes. The focus of patient education should be showing him how to use mHealth for better disease management.
Identify Teaching Areas related to the mHealth app
- Information: the patient will be taught the purpose of the MySugr app, its essential features, and how it is designed to support patients in diabetes management. The app guides patients on carb counting, continuous glucose monitoring, and bolus dose estimations ((mySugr GmbH, 2021).
