As nurses, we work with patients, providers and healthcare team members whose native language is not English. This diversity may lead to challenges with communication. Consider how to apply cultural awareness to nursing practice.

Happy Week 2!

This week we will be reflecting on diversity, communication, and cultural awareness in nursing practice. I look forward to our discussion on this process.

This week’s graded topics relate to the following Course Outcome (CO):

CO 2: Communicate effectively with culturally diverse individuals, families, and communities. (PO 3)

Communication is key in my area of practice, as I work with patients who are anxious, depressed, using substances or with a multitude of psychiatric problems/conditions. As you can imagine, any type of bias expressed can lead to negative consequences such as distrust which means the patient will not openly talk about their issues or needs. Poor communication can also cause patients to not feel respected, so they may elect not to return for follow-up.

I would like to relate a cultural miscommunication,

One day, I was in the nursing station, documenting a wound found of a patient and I heard a door slam shut. I looked up to see what had just occurred. Cena came out of my patient’s room and her face was red. I asked Cee what happened? Cena stated, “I was trying to help Ms. J. put on her TED hose and she called me a pig!” “A PIG!!!” Cena was clearly upset. I said to Cena, “I am not sure I understand, why would she call you a pig?” She said, “I don’t know, I was helping her, and she called me a pig!” Then she said, “I just need to go take a few minutes to calm down” and she walked off.

I walked into Ms. J’s room. I said to Ms. J, “What happened with your occupational therapist?” She looked confused and flustered. She said, “Honestly, I don’t know, she got angry at me because I couldn’t get my TED hose on.” I told Ms. J that didn’t sound like Cena, and then I asked her to tell me exactly what happened. Ms. J said, “I was pulling and pulling on the hose and she said to me, ‘here, do it this way’ and I said, ok, I’ll try, but as hard as I tried, I couldn’t get them pulled up” I said, “hmmm, that doesn’t sound like anything that would make someone mad. Did you say anything else?” She said, “Well, I told her, ‘you can’t make a silk purse out of a sow’s ear.’ I could not help but smile, I knew exactly what had happened. I told her I understand now. I explained to Ms. J that Cena was not from our country and that English was a second language for her, so she may not understand that saying. I further explained to Ms. J that Cena thought she was calling her a pig. Ms. J. looked embarrassed and turned red in the face sputtering, “Oh my, noooo, no (shaking her head vehemently) that isn’t what I meant at all!” I told her I understood, and I would go speak with Cena.

I found Cena in the nurse’s lounge. I told Cena I needed to explain what happened with the patient and I told her what the aphorism meant (for those of you who have never heard it before, it means that you can’t make something work, if you do not have what it takes to do it). Cena immediately calmed down and said to me, Ms. J must think I am a crazy person. We went back to the patient’s room and cleared things up.

This could have ended badly, with another occupational therapist being assigned to the patient and the patient left not understanding what had happened but instead it ended with each understanding a bit more about the other.

As for how this could be prevented in the future, as a present for a new diverse employee, I could give them information on the area, local aphorisms and other information that might be helpful in working with Appalachia patients.
