Psychiatric patients with multiple chronic conditions face unique challenges when managing their medications effectively. These patients often have complex medication regimens that can be difficult to manage. They may also have difficulty understanding the purpose of their medications, how to take them correctly, and the potential side effects. As a result, these patients are at risk for medication errors, non-adherence, and poor health outcomes. This paper proposes a DNP change project focusing on implementing medication management protocol to enhance care delivery for psychiatric patients with multiple chronic conditions in mental health centers.

World Health Organization (2019) defines medication management protocol as written instructions that enable nurses or midwives to safely provide and administer a specific medicinal product within specified clinical circumstance. It involves various activities, including prescribing, dispensing, administering, monitoring, and evaluating medications. It aims to optimize therapeutic outcomes while minimizing the risks and complications associated with medication use (Okobi et al., 2022). Additionally, medication management protocol ensures patients benefit from tailored medication regimens that address their unique combination of mental health symptoms and chronic illnesses, resulting in better symptom control, enhanced functioning, and overall well-being.

Furthermore, most psychiatric patients with multiple chronic conditions often struggle with medication adherence due to cognitive impairments, medication regimen complexity, and side effects (Kane et al., 2020). Implementing medication management protocols provides a framework for healthcare professionals to educate patients about the importance of adherence and help them develop strategies to overcome barriers by incorporating patient education, medication reminders, and regular follow-up.

As Fernandez-Lazaro et al. (2019) assert, medication protocols facilitate interdisciplinary collaboration among mental health professionals, primary care providers, and specialists involved in the patient’s care. This integrated approach results in improved patient outcomes, reduced healthcare costs, and increased patient satisfaction. The implementation of a medication management protocol for this DNP change project will address the challenges faced by psychiatric patients with multiple chronic conditions by providing clear instructions and guidelines for healthcare professionals to safely administer and monitor medications.
