Osteoporosis is a bone disease that occurs when there is an imbalance between the deposition of calcium to form the bone fracture and the breakdown of the bone to release the calcium. It can therefore be a result of increased bone destruction or decreased bone formation. It is one of the most common musculoskeletal conditions and the risk factors include positive family history, Caucasian race, advanced age, thin build, dietary factors, liver or kidney disease, early menopause, and smoking. An individual who has either of these factors and presents with bone pain should be suspected of having osteoporosis.

Osteoporosis has been observed to occur more commonly in individuals whose family members have the condition. For instance, Pouresmaeili et al. (2018) report a higher incidence of osteoporosis in individuals whose twins have developed the condition. Caucasians also have a higher risk of developing osteoporosis; 50% higher than blacks and 25% higher than the Asian population although the reason for the predisposition is unknown (Ibrahim et al., 2019). Advanced age is also an identified risk that increases bone resorption but reduces bone formation as a thin body is a risk due to reduced bone density. Dietary factors such as low calcium and vitamin D intake or increased alcohol consumption also contribute to the condition. Further, kidney or liver diseases that interfere with the formation and storage of vitamin D respectively, result in osteoporosis because vitamin D facilitates GI absorption of calcium for bone formation (Pouresmaeili et al., 2018). Such patients may therefore require long-term vitamin D and calcium supplementation.
Nurses participate in the prevention and management of osteoporosis by offering patient education. Such education will focus on identifying existing risk factors for osteoporosis and enlightening the patient on these risk factors (Wozniak et al., 2020). The nurse thereafter advises the patient on effective measures for the prevention of osteoporosis including avoiding alcohol intake and cigarette smoking, adopting a healthy diet, and engaging in physical exercises to facilitate bone formation (Ibrahim et al., 2019). The nurse would also provide necessary supplements and advise the patient to adhere to supplementation therapy. Their involvement, therefore, improves the management of osteoporosis.

She has not taken her antihypertensive medications or medications to control her heart failure for 3 days. Today, she has been admitted to the hospital ICU with acute decompensated heart failure and acute exacerbation of COPD.

Is very anxious and asks whether she is going to die.
Denies pain but says she feels like she cannot get enough air.
Says her heart feels like it is “running away.”
Reports that she is exhausted and cannot eat or drink by herself.
