The reflective journal this week focused on population health concerns and health disparities. Indeed, the preceptor and I have discovered a raft of issues that impact population health and are a source of health disparities in the selected population (MacKay et al., 2021). Population health concerns in this case include increased severity of patient falls emanating from medications and limited mobility devices as well as sufficient levels of skilled healthcare providers in the facility (Raymo et al., 2022). The elderly individuals in the selected facility are concerned about their safety due to the limited number of skilled nurses and nurse assistants serving them. The implication these individuals believe that they are not getting sufficient resources to cater to their needs.

Another discovery is the increasing levels of health disparities in this population. Health disparities affect communities due to the differences in health status among different groups and are linked to social, economic and environmental issues and disadvantages. Individuals and groups adversely affected by disparities include those facing greater hurdles to health based on their racial, religious and socioeconomic statuses (Schoberer et al., 2022). As such, we discovered that many elderly patients in this community do not have sufficient resources to cater for their needs. The facility provides care based on different levels or plans for different patients. For instance, while other patients can access care in private wards due to their resources, others have to do with a common living area that has limited provisions like number of skilled nurses assigned to a certain level of patients.

My personal strengths in addressing these issues include teamwork, engagement of patients and their families, effective communication, and working closely with others to generate long-term interventions that benefit individual patients. My weakness may include possible biasness, negative approaches to situations as they arise, and increased reliance on technologies like telehealth (Heng et al., 2022). More fundamentally, improving outcomes in this population warrants more resources like having sufficient workforce levels and skilled employees as well as finances to develop better interventions for the residents in the nursing home.
