Topic 7 DQ 1

Evidence-based practice (EBP) entails the integration of research evidence with a healthcare professional’s clinical expertise and also advocates for the inclusion of patient preferences and values, which promotes individualized care (Grove & Gray, 2018).  There are several steps in the evidence-based research process. According to Luthans et al., (2021), the first step is asking clinically guided questions. This step involves the formulation of PICOT questions. PICOT is instrumental in directing the development of the research question. The second step entails the search for relevant evidence. This step enables researchers to look for evidence using scholarly databases such as PubMed and Cochrane Library to determine if the clinical problem at hand is suitable for the EBP change proposal.  The third step entails analysis of the evidence through the completion of an in-depth investigation of the evidence collected before concluding. The fourth step is the incorporation of the evidence into clinical practice. This step characterizes the conclusion of the research and the researcher determines whether or not the evidence generated can be applied to the clinical practice.  The fifth step is the evaluation of outcomes to determine whether or not the expected results are attained. The final step is the distribution of outcomes for use in clinical practice (Luthans et al., 2021). These steps are systematic and all of them should be integrated into the research process.

In the initial stages of an EBP project, nurses in the leadership specialty tend to locate sources of information from various databases to help them determine whether or not a practice problem is appropriate for an EBP change proposal. Specifically,  nurses can use the CINAHL database, which is a leading source of full-text nursing and related health journals globally (Hopia & Heikkilä, 2020). The database is majorly used by researchers and students.  The other source of information is PubMed. PubMed is freely available to the public. Using PubMed enables researchers to access over 30 million citations of biomedical texts.

Topic 7 DQ 2

Advanced registered nurses take part in research, enhance healthcare services and help clinicians promote patient health outcomes.  As such, advancing professional integrity and ethical standards when conducting research are critical roles of advanced registered nurses.  Some of the major ethical issues in conducting research that impacts advanced registered nurses include preventing participants from harm, ethics of caring, protection of participants’ dignity, and perception of society about human rights (Hasan et al., 2021). Other ethical issues that advanced registered nurses may experience when conducting research include contradiction between the values of researchers concerning scientific investigations,  the participants’ values or beliefs and patient privacy and confidentiality, and obtaining informed consent of participants (Larkin et al., 2019).  Getting informed consent from patients is a key obstacle for nurses during research.  Nurses may face challenges in obtaining informed consent from mentally challenged subjects, elderly people, children, and critically ill patients because they may not fully understand what is needed in them to participate in the research (Hasan et al., 2021).

Patient advocacy is among the leading roles of advanced registered nurses performing research. The nurses have the responsibility to advance ethical conduct during the research process.  Therefore, as patient advocates, advanced registered nurses have a critical role in ensuring that participants are well-conversant with pertinent information about the research, research opportunities, and their rights as participants in the research (Kalaitzidis & Jewell, 2020).  Advanced registered nurses must also be aware of what clinical research entails and what makes research ethical.  Determination of these factors is critical for the advanced registered nurses to establish if action is required to ensure that research is performed in the right way and the participants are treated appropriately and ethically.
