Based on the PICOT you developed for NUR-550, summarize the intervention you are proposing. How does this support the population of focus, your setting, and role? Justify how the problem you selected to investigate is amenable to a research-based intervention using the PICOT format. Include your PICOT statement with your response.
Sample Answer
Alzheimer’s patients were disproportionately affected by COVID-19 (Azarpazhooh, 2020; Wang, 2021). While there is a biologic/genetic component to the mortality rates (Wang et al., 2020), it is unknown whether the quarantine induced social isolation contributed to mortality rates. Since COVID-19 vaccines have high efficacy (CDC, 2021) and have increased the safety of the elderly in community settings, it has become more common for facilities to allow Alzheimer’s family member act as a caregiver and advocate.
The importance of family/friends to Alzheimer’s patients can not be denied. During the initial COVID-19 lock-downs of community settings and long-term care, dementia patients neuropsychiatric symptoms and confusion increased due to the dramatic change and loss of their loved ones (Boutoleau-Bretonnière,2020). Therefore, during the pandemic, caregivers were eventually allowed into the facilities for compassionate visitation.
The question facing the community as it faces future pandemics and lock-downs, is how to keep the Alzheimer’s patients safe without the dramatic social isolation from their family caregivers. Therefore, like paid caregivers and medical staff, family caregivers should receive basic infection control training.
The PICOT is defined by:
Will educating Alzheimer’s’ caregivers in disease transmission control help reduce community acquired infections in Alzheimer’s patients living in community settings (i.e., long term care or assisted living) over a 3-month period?
Population: Alzheimer’s patients
Intervention: disease transmission education
Comparison: education complete versus not
Outcome: reduce community acquired infections (i.e. COVID-19, pneumonia, etc.)
Time period: 3 months
By educating family caregivers, it adds to the resources of the community settings. In the case of long-term care, the patient receives better care from their family member with staff support and reduces demands on staff time. This reduces staff stress and caseload while increasing the availability of staff to other patients.

Obesity is a growing health issue in the United States that can contribute negative outcomes with to other health conditions. It is seen as primary precursor to unhealthy outcomes and conditions as well as a major risk factor to chronic conditions.
Reducing obesity within populations can be an important intervention in increasing the overall health quality and increase preventative efforts. These efforts would include nutrition changes, weight loss programs, and physical activity programs (Pascual et al., 2019).
Based on this, my PICOT includes providing information and education about nutritious diets that would aim to reduce body mass index and produce a decrease in obesity rates. My formulated PICOT is as follows: In obese adults (P), does an education of nutrient dense diets (I) compared to individuals with no provided education (C) achieve a decrease in Body Mass Index (BMI) (O) within three months (T)?
Research based interventions is important in my role as an advanced registered nurse. Increasing the quality of health and wellness in the population is part of the obligation advanced nurses have to the public or populations. In the case of obesity, interventions to decrease obesity rates could lead to better health outcomes, prevention or treatment of chronic conditions and public health maintenance all of which can be advocated through advanced nurses (Al-Nimr, et al., 2020).

Patients with obesity are at major risk for developing a range of comorbid conditions, including cardiovascular disease (CVD), gastrointestinal disorders, type 2 diabetes (T2D), joint and muscular disorders, respiratory problems, and psychological issues, which may significantly affect their daily lives as well as increasing mortality risks.
A relatively small and simple reduction in weight, for example, of around 5%, can improve patient outcomes and may act as a catalyst for further change, with sustainable weight loss achieved through a series of incremental weight loss steps (Fruh, 2017).

Obesity is a critical health concern in the United States, especially affecting the minority populations. It is considered one of the greatest risk factors for many chronic illnesses including diabetes, hypertension, and cardiovascular disease (Schetz et al., 2019). Management of this condition does not only prevent an individual from contracting the long-term diseases but also improves quality of life.
Your choice of intervention provides the basis for a research that provides a comparative view between compliance and non-compliance to the lifestyle standards. Educating patients on nutritional guidelines and weight loss programs are essential considerations for decreasing the Body Mass Index (BMI), which in return reduces the risks for developing the chronic health conditions.
As you have stated, advanced practice nurses are obligated to improve population health through the adoption of research-based intervention. Your proposed intervention creates an opportunity for using evidence to address obesity from a population-based setting.

Research studies are systematic and objective and thus follow a specific structure that informs their design, population of interest, and data collection and analysis and information dissemination processes. Evidence-based practices are also systematic, and their implementation is an organized, complex, and resource-intensive process that requires precision to prevent resource wastage and enhance their success. These processes could benefit from change frameworks or models.
Models and frameworks are integral in evidence-based practice because they give a solid, evidence-based path for action (Speroni et al., 2020). Change models help researchers maintain focus on the project/research and its objectives and prevent deviations from these objectives. They assist with decision-making as they categorize the changes depending on the theory used.
The frameworks provide precision in decision-making and help with precision as they outline assistive steps that help implement change or carry out interventions (Fineout-Overholt, 2022). They also simplify the complex change process hence their significance. Their reliability is grounded on advanced research and experience backing these theories.
Selecting the wrong change framework has various implications in research and can lead to biased results. It makes meeting the set objectives and the desired outcomes challenging (Nilsen, 2020). Biases such as selection bias occur when the wrong framework is selected. It alters research implementation and data collection, analysis, and dissemination.
Nilsen (2020) notes that selecting the wrong framework makes reading and understanding research difficult because they do not resonate. The research findings are thus often weak and hardly usable due to the mismatch in the concepts in the research and the theory or framework. Thus, selecting the suitable model or framework impacts the results and quality of outcomes.

Various change management models can be used. These models include the IOWA model, Lewin’s 3-stage change theory, McKinsey 7-S, Kotter’s 8 Steps, Nudge theory, Settlers, and the ARCC model. Some models, such as the IOWA, are suited for implementing evidence-based practice, while others, such as the McKinsey 7-s, are suitable for implementing and testing new practices. This essay reviews two change models and selects one for this project.
The Two change models of interest are the IOWA model and Kurt Lewin’s change theory. The theories selected are based on the fact that the project is implementing an evidence-based healthcare practice. The IOWA model, developed in 2015, aims to implement evidence-based practices.
The project guides the project managers to implement evidence-based practices, unlike Lewin’s change theory which focuses on overall organizational changes, including evidence-based practices (Hussain et al., 2018). Kurt Lewin’s theory focuses more on preventing and managing change resistance, while the IOWA model focuses on developing evidence-based practices and implementing and evaluating them.
Speroni et al. (2020) note that the IOWA model is complex and is one of the most applied change models in healthcare to implement evidence-based practices. The IOWA model contains pilot testing in one of its steps. However, Lewin’s theory only implies its possible use, making Lewin’s theory suitable for changes that do not require a pilot study (Collaborative et al., 2017). These two models are widely applied in healthcare to lead change and implement evidence-based practices.
The best model for the change model is Lewin’s change management model. Most of the project’s interventions and validation as an evidence-based project are complete; hence, the model will be vital in implementing and evaluating the EBP. The research in earlier assignments revealed that BMCA is a hardly implemented technology, and some of the reasons include nurses;’ lack of knowledge (Bedot et al., 2019).
The project’s main aim is to ensure BMCA technology is implemented with optimal resource use, is accepted, faces less resistance, and is embedded in the organizational culture to cement its effectiveness. The model will help evaluate the EBP before its implementation and guide it to achieve the desired results.

CAT: What is the difference between a qualitative and a quantitative research design?
Research designs are the strategies that help integrate study components to logically and coherently address a research problem. Strijker et al. (2020) note that research dictates data collection, measurement, analysis, and dissemination. The research design is influenced by the study’s research questions and objectives. Qualitative and quantitative research studies vary based on these parameters. Qualitative research designs are subjective inductive processes, while quantitative research designs are deductive objective research studies. Due to their nature of testing, quantitative studies have statistical data and thus utilize data analysis tools and statistical tests. Qualitative research designs utilize thematic and content analysis because they deal with textual data, while quantitative studies use numeric data (Mehrad et al., 2019). The nursing practice of interest is medication errors, and the evidence-based intervention selected is Barcode Medication errors. Prevalence and burden data are quantitative and are based on the number of medication errors. Any research study’s objectives and research questions influence the design because studies attempt to answer study questions and meet the research objectives.
The study’s objectives can only be studied using statistical means by evaluating data from the hospital database. The prevalence of medication administration errors compared to other medication errors and the changes in these errors will be vital in determining the effectiveness of BCMA (Williams et al., 2021). The data will thus require statistical evaluation, hence the need for a quantitative research design.
Bloomfield and Fisher (2019) state that a correlational quantitative research design is best suited to determine the relationship between variables and is thus best suited for this study. Data collected for this project will include medical records on medication errors in the facility and the collection of raw data (medication errors, including near misses and sentinel events) related to medication administration collected throughout the project. For this project, a quantitative design will be the most suitable to help collect data and evaluate the proposed project’s effectiveness. Data for this project will be quantitative hence the design’s selection.
Document review/secondary data collection methods and surveys utilizing quantitative research designs will collect and analyze data for the project. Document reviews utilizing healthcare records help professionals avoid time and resources wasted in data collection because the data is readily available (Melnyk & Fineout-Overholt, 2022). Data from the healthcare database on medication errors is the best to use in this quasi-experimental study and results in medication administration error differences pre-an post implementation of barcode medication administration technologies. Selecting the right research design will help with objective achievement through adequate planning.

The primary interest of the project is to determine changes in medication errors. Statistical tests in research are selected based on the objectives. Statistical. Data evaluating the relationship between variables (causal effects), data changes over time, and hypothesis testing require specific statistical tests. Tests such as ANOVA, t-test, and chi-squares help understand the variables’ changes and their relationships (Rebekah & Ravindran, 2018).
Zheng et al. (2021) note that the BCMA technology targets medication administration errors by confirming all medication administration rights at the dispensing point. The technology may affect other medication errors at dispense and prescription, but the primary interest is medication administration errors. The interest statistical test must be able to test changes in variables of time/ at different times.
The selected statistical test is the ANOVA test. The test helps evaluate the changes in variables concerning each other. They test the significance of results and thus help reject or accept the null or alternate hypothesis. The project aims to determine if barcode medication administration reduces medication administration errors. The statistical cost interest should help determine the relationship between BCMA administration and changes in medication errors. Data before and after BCMA should be compared to determine their similarities and differences.
The ANOVA test will be vital because data will be collected and analyzed at different times. The analysis will help determine the differences in medication (Rebekah & Ravindran, 2018). The ANOVA test is used when testing a hypothesis and evaluating the effectiveness of interventions. The ANOVA will help test the changes in the various variables over time. The changes in the variables, medication error rate, and their relationship to other medication errors will help evaluate the effectiveness of the interventions.
