The Selected Model and the Relevance to the Project

The proposed model for the project is the Iowa Model. This model is key in helping healthcare providers transform results from research efforts into clinical applications to enhance patient quality outcomes. One of the aspects of this model that makes it relevant to the project is that of triggers which entail external and internal data that is used in identifying clinical problems (Tucker et al., 2020). The Stages in the Change Model

The selected model has eight steps that are chronologically followed. The first step involves problem identification as triggered by the internal and external data, followed by an assessment of the identified problem is a priority to the unit, department, and organization. The next phase of the model involves formulating a team and assessing the existing practice (Green, 2020). The fourth step entails gathering and analysis of research connected to the targeted practice change literature critique. The next phase of the project entails determining if existing research is sufficient to warrant practice change implementation. This is followed by the implementation of the change and, finally, an evaluation of the results (Green, 2020). The concept map for the Iowa model has been included in the appendix.

Application of Each Stage in the Proposed Implementation

            As earlier indicated, the eight stages of the model are chronologically applied to help in the change implementation. In the first phase, the identification of patient falls was triggered by the patient falls data at the facility, which was found to be above the national benchmark. As such, it was identified as the organization’s clinical problem. The problem and the data that supports the push for change and an assessment and alignment of the organization’s priorities help in improving the chances of success (Green, 2020). The next phase will entail coming up with a team to help assess the present practice and locate clinical practice guidelines, and evidence-based literature for patient falls. The selection of unit champions to spur formulation, development, implementation, and evaluation of the project is key. The next phase will entail piloting the proposed change where the Fall Tailored Interventions for Patient Safety (TIPS) Toolkit will be tested for its efficacy in preventing patient falls before evaluating the results or findings to help inform the next course of action.

Conclusion

Patient falls in the patient care settings have been shown to result in various adverse patient outcomes. Therefore, this project proposes to use Fall Tailored Interventions for Patient Safety (TIPS) Toolkit to help control patient falls. As part of the implementation, the Iowa model has been chosen as the model to help guide the practice change to help improve patient outcomes.
