History and physical examination in the geriatric population is unique as it is multidimensional and aims to evaluate the patient’s functional ability, physical health, cognition and mental health, and socio-environmental circumstances. As a result, a geriatric medical history has detailed information regarding his past medical history, including any chronic illnesses and the care provided to him (Thurner et al., 2020). In addition, the history should cover an extensive review of prescription and over-the-counter drugs, vitamins, herbal products, and immunization status. An additional inquiry into the patient’s psychological health and social support is also necessary. A physical examination for these patients should include a mini-mental status exam (MMSE) to evaluate the patient’s orientation, immediate and delayed memory, language, and attention (Salis et al., 2023). The MMSE aims to identify neurological deficits, especially when a score is below 25 out of a possible 30. NUR 634 Topic 16 DQ 2

Non-communicable diseases (NCDs) are the most common causes of morbidity and mortality among the geriatric population. As a result, the NCD screening tool is crucial in identifying several NCDs, their risk factors, and symptoms. Common diseases screened include hypertension, diabetes mellitus, hyperlipidemia or dyslipidemia, coronary artery disease, cancers, and chronic respiratory illnesses (Gupta et al., 2021). The tool contains questionnaires that inquire about various risk factors such as poor diets, physical inactivity, tobacco use, excessive alcohol consumption, obesity, hypertension, family history, and raised blood lipids and glucose. The cancers screened in the tool include breast, colorectal, prostate, and lung cancers that are more prevalent in the geriatric population. The overall goal for screening for NCD is to allow early identification of NCDs and their risk factors and to reduce related complications, morbidity, and mortality. The screening tests for the above diseases include regular blood pressure and sugar monitoring, lipid profiles, mammography, chest radiography, serum prostate-specific antigen, and fecal occult blood testing.
