Tell us about a healthcare program, within your practice. What are the costs and projected outcomes of this program?
Pressure injury, also known as pressure ulcers, bed sores, ore pressure sores, are localized injury to the skin and/ or underling tissue usually over a bony prominence (such as the elbow, heel, hip, shoulder, back, and back of the head), as a result of pressure, or pressure in combination with shear and friction. Pressure injuries are serious medical conditions and one of the important measures in the quality of clinical care in nursing homes (Liu et al., 2019).
Being a part of an organization that has eight skilled nursing facilities with the same upper corporate management, they do not function the same way. I have been the Wound Care Certified nurse in my facility in the past five years. I see the number of pressure injuries and 60 percent are acquired in house. Our focus is to develop a health care program to better implement and decrease the in-house pressure injuries. This healthcare program had tremendous success within my facility and acquired in-house pressure injuries have been reduced. I will be presenting to the other seven skilled nursing facilities. This program will focus on decreasing the acquired in-house pressure injuries by making sure that residents are being identified as high risk early, and being treated appropriately

Who is your target population?
The target population is residents in skilled nursing facilities. Residents aged 64 years and under were more likely than older residents to have pressure injuries. Residents of nursing homes for a year or less were more likely to have pressure injuries than those with longer stays (Gupta et al., 2020). One in five nursing home residents with a recent weight loss had pressure injuries (Liu et al., 2019).
What is the role of the nurse in providing input for the design of this healthcare program? Can you provide examples?
The role of the nurse in the health care program is to use the Braden Risk Assessment upon admission, weekly and as needed when there is a change in condition (Lim et al., 2019). Nursing will play an important role by working with other health care professionals to ensure that residents who are at high risk in developing pressure injuries are identified and interventions are placed, and to receive proper treatment when they have an existing pressure injury.
Example of the design will be as follows:
- Nursing will familiarize themselves with the use of risk assessment scale-Braden risk assessment
- Nursing will be familiar with comprehensive skin assessment to identify changes in intact skin
- Nursing will be familiar with interventions and treatment for residents who are identified at risk in developing pressure injuries
- Interdisciplinary team will conduct wound rounds weekly and will meet to ensure that residents who have a pressure injury upon admission, or high risk in developing one have proper interventions in placed.
What is your role as an advocate for your target population for this healthcare program?
As an advocate, the target population will be the residents in the skilled nursing facilities. As the wound care certified nurse, I will be teaching, guiding, consulting, and initially head the wound care team at the facility where I work at (Gupta et al., 2020). Do you have input into design decisions? How else do you impact design?
As the Wound Care Certified Nurse, I will make sure that every resident who has pressure injuries upon admission, or have acquired it in-house, is at high risk for developing pressure injuries, and those who recently had a change of condition will be assessed and evaluate if the proper interventions and treatments are in placed.
What is the role of the nurse in healthcare program implementation?
The role of the nurse in the health care program implementation is to come up with a program that focuses on reducing the number of acquired in-house pressure injuries because of the negative impacts of pressure injuries on residents (Gupta et al., 2020). Adequate prevention starts with the accurate identification of residents at risk for pressure injury development. To identify residents at risk, a structured approach should be used (Lim et al., 2019).
Who are the members of a healthcare team that you believe are most needed to implement a program? Can you explain why?
Nurses are the main focus because they are the front-line staff that sees the residents every shift, every day. The practitioners are also a focus since they are the one prescribing the treatments and other interventions based on the clinical guidelines developed with the nursing home (Gupta et al., 2020).
The facility has decided to modify the protocol around certain interventions such as: low air loss mattress when a resident scored a high risk based on Braden scale for example, and utilization of supplements such as Multivitamins with minerals, Vitamin C, protein supplements, adding protein and calories in their diet as needed (Gupta et al., 2020).
How does this role vary between design and implementation of healthcare programs? Can you provide examples?
Nurses will follow the toolkit Pressure Injury Prevention and Treatment which we have educated them on and implemented (Gupta, et al., 2020).
The toolkit form will have the Braden Risk Assessment form, including interpretation of the results to at risk, moderate risk, high risk, very high risk.
Whichever category the resident falls under, there will be interventions that the nurses can initiate.
In the toolkit, there will also be pictures and descriptions of pressure injury staging guide, wound assessment findings, and how to document properly.
For example, when a new resident is admitted, the nurse can go to this toolkit and see step by step what she needs to do. After assessing the resident, the nurse can complete the Braden scale. Whichever category the resident falls under (at risk, moderate risk, high risk, very high risk), the nurse can see the interventions that are recommended for each category. In conclusion, strong leadership is critical if the vison of a transformed health care system. In the nursing field, we must lead and develop health care programs throughout the health care system. Leadership varies from bedside care to the boardroom, and nurses need to partner with other health professionals to deliver high quality care (Busche & Marshak, 2017).
