Quality improvement (QI) is a systematic, formal approach to analyzing practice performance and efforts to enhance performance. QI employs a systematic change method and strategies to improve patient experience and outcome (Hamilton et al., 2020). I would identify a QI practice gap by identifying a problem in patient care delivery or patient outcomes. I would then plainly define the outcome that needs to be enhanced, identify how the process outcome will be measured, and create a plan for executing an intervention. I would also create a plan for collecting data before and after the intervention, which will help measure the impact of the QI intervention.
In my current practice setting, I have identified a practice gap in the discharge of patients from the inpatient psychiatric unit. The unit has suboptimal discharge planning, which is linked to poor understanding of and lack of compliance with medical instructions among patients. This leads to decreased treatment compliance, reduced patient safety, increased readmissions, and poor patient satisfaction. I can implement a QI project on safe discharge, which entails discharging patients from the hospital with a plan for continued care after leaving the facility.
The tools I may adopt to address the practice gap include the PDSA and Six Sigma models. The PDSA is a QI process applied to improve a practice or make a change. It is a cyclical four-stage problem-solving process involving dividing a task into steps, evaluating the outcome, improving it, and evaluating it again. The Six Sigma model offers organizations tools to enhance the capacity of their business processes (Niñerola et al., 2020). The outcome is increased performance and reduced process variation that contributes to a reduction in defects and improved employee morale, profits, and quality of services/products.
