Journal Entry

Strengths and Challenges

As far as pediatric clinical experiences are concerned, what most excites me is the opportunity to be able to interact with patients who cannot explain themselves very well as to what is bothering them. This is exciting for me because it will make me use clinical acumen and critical thinking skills in arriving at correct diagnoses and prescribing the correct treatments and investigations or assessment tools. The challenge that concerns me is the fear that the parents and guardians of the children may see that I am not yet quite competent and refuse that I see their child. This challenge is real because one weakness I have is that I have never worked with children before. However, my strengths include that I have done well in theory in class and so I should not have a problem applying the theory to practice. My other strength is that I speak both English and Spanish and so I will have an easy time communicating with a majority of the parents and their children.

Family Roles

My personal definition of a family is a basic social unit that is closely held together by blood and social links. Family roles that might impact the assessment of a child include that of a father as the protector and provider, mother as the home maker and problem solver, and siblings as companions. Some of the challenges of working with families include language barriers, cultural incompetence on the part of the nurse, and strong beliefs held by the parents about some interventions such as vaccinations. Thus, for parents that do not want to vaccinate their children, I would adopt a friendly and understanding approach and first work to gain the parent’s confidence and trust. This may take some time. After I earn their trust, I will gradually and tactfully explain the reason why their child needs vaccination and why it is safe. I will encourage them to tell me all their worries and will answer them only using facts. Most importantly, I will let them know that the power to decide is in their hands and no one can forcefully do what they do not want done to their child. This ethical principle of autonomy (Haswell, 2019) is important and I must uphold it.

Cultural Considerations

Cultural competence is an invaluable asset that the provider must have. This means understanding that we may have different views of the world from that of our patients depending on the part of the world in which we were born and grew up. It does not mean that the culture of either of us is better than that of the other. This understanding of multiculturalism plays a crucial role in the successful evaluation and assessment of children in the hands of their family. Cultural understanding is also part of holistic care based on transcultural nursing as explained by the nurse theorist Madeleine Leininger in her Transcultural Nursing Theory (Gonzalo, 2019). Cultural sensitivity is therefore going to be key in my success in the practicum.

Nursing Theory

The nursing theory that will guide my practice with pediatric patients is that of the nurse theorist (Wayne, 2020) Dorothea Elizabeth Orem known as the Self Care Deficit Theory or SCDT (Borji et al., 2017). This is a middle range theory that states that a patient who cannot perform the functions of daily living on their own (such as children) have a self care deficit that needs to be filled (Risjord, 2018). The responsibility of filling that gap falls on the nurse.
