I plan on involving healthcare practitioners from the UTMB’s health clinic throughout the implementation of this health intervention plan. I will also engage the hospital’s management and healthcare practitioners in evaluating the plan’s progress. I understand the HIPAA act does not allow me to access patients’ health information without permission from the relevant stakeholders. Therefore, I will request the facility for a report on different health metrics associated with diabetic patients and use the information to determine whether my intervention plan is working.
