A good percentage of patient-physician experiences are often considered difficult by the physicians involved. There is a range of patient characteristics that indicates the probability of difficult experiences. They include the availability of anxiety or depressive disorders, somatic symptoms, and more symptom harshness. However, not every difficult encounter can be attributed to patients. Sometimes physician factors such as stress and burnout, fatigue, or attitude can lead to circumstances where physicians are blamed for the difficult encounters. Other factors that can cause difficult patient-physician encounters include cross-cultural challenges, language barriers, and the need to break bad news (Jackson et al., 2021).

The identified challenging patient situations and behaviors entail dealing with an angry, terrified, or resistant patient. These patients can be identified by various signs such as restricted patterns of breathing, furrowed brows, compressed fists, squeezing of the hands, and tips from colleagues that something could be wrong with the patients. When physicians notice these signs, it is imperative to strive to unearth the origin of the problems for the patient and be attentive to the emotions of the patient regarding the clinical issue in question (McCarthy et al., 2018).  Physicians should not be involved in the problem but define boundaries and determine the appropriate time to respond to the situation empathically. Physicians should reassure the patient using reflective statements that indicate empathy for the feelings of the patient. This should be followed by a discourse about the right course to address the situation.                For instance, when a physician notices that a patient is afraid about the physical examination or history taking, the patient should be inspired to talk about it and evaluate whether the fear is suitable to the situation. This is critical in creating a context for the fear and enables the patient to address it more constructively. Moreover, when a physician is dealing with an angry patient and notices probable harm, assistance should be sought from law enforcement.
