Strategic planning is a process designed to achieve goals in a dynamic competitive environment through the allocation of resources. It is a continuous, systematic process of making decisions today with the greatest possible knowledge of the impact in the future. The steps in strategic planning include scanning the environment, strategic formulation, identifying strategies, implementing strategies, and strategy evaluation (George et al., 2019). The purpose of this paper is to outline a strategic plan that will address the nursing shortage in the behavioral health unit at Chelsea Hospital.

Strategic Plan Concepts

The strategic planning process will be driven by a team that will include the Chief Nursing Officer (CNO), Director of Human Resources (DHR), Hospital administrator, the nurse-in-charge of the behavioral health unit, and two registered nurses (RNs) working in the unit. The CNO is at the top-level management of the organization’s hierarchy and is also the hospital’s vice president. The CNO was included because she will spearhead personnel management with regard to the nursing shortage, which means implementing the recruitment, hiring, and retention processes.

The DHR is also at the top-level management in the organization’s hierarchy. The DHR was included in the team because he is involved in recruiting, hiring, and retaining staff in the hospital. In addition, the hospital administrator is a top-level manager responsible for organizing and supervising the hospital’s health services and daily activities. The administrator was included because she monitors departmental and staffing budgets and develops work schedules. The nurse-in-charge is a low-level manager in the hospital. She was selected because she is responsible for managing the unit, overseeing staff management, patient care, and daily operations. Lastly, the RNs are involved in providing direct patient care and are the most affected by the nursing shortage in the unit. They were selected to provide solutions on how to best address the nursing shortage in the unit.

Priorities

The two priority solutions to addressing the nursing shortage at Chelsea Hospital are investing in the professional development of nurses and eliminating mandatory overtime. Mlambo et al. (2021) explain that staff turnover intention is imminent based on the perceptions of staff development and is closely connected to an employee’s intentions to leave. Therefore, employers can contribute to staff career development and retention by implementing organizational programs and activities aimed at career development (Hu et al., 2022). Mandatory overtime is associated with fatigue and burnout among nurses, which decreases their job morale and satisfaction and increases their intention to leave and turnover rates (Kelly et al., 2021). The nursing shortage will be addressed by eliminating mandatory overtime in the hospital, which will create a healthy work environment, especially for nurses more vulnerable to burnout. Besides, this will create a positive practice environment that acknowledges work-life balance, in which there is respect for nurses’ safety.

The priorities relate to leadership since leaders have a responsibility to help employees continuously improve their knowledge and skills through professional development programs. Leaders also have a role in empowering employees, and providing professional development opportunities will be a major way to empower them. In addition, leaders have a role in creating healthy working environments, which will be met by eliminating mandate overtime policies that cause fatigue (Hu et al., 2022). Furthermore, the priorities relate to the Person-Centered Framework by McCormack and McCance (2017) since professional development programs will promote professional competency and increase nurses’ commitment to the job. Besides, eliminating mandates over time will improve the practice environment, promote effective staff relationships, and enable nurses to be sympathetically present to patients.  

Conclusion

The strategic planning team will comprise the CNO, DHR, Hospital administrator, the nurse-in-charge of the behavioral health unit, and two RNs. Priority solutions are investing in nurses’ professional development and eliminating mandatory overtime.

Professional development programs are associated with overall job satisfaction and retention. Eliminating mandatory overtime will reduce burnout, increase job satisfaction, and reduce turnover.

Deanna: Cheryl, thank you for agreeing to meet with me and answer a couple of questions about healthcare, making a difference and the nursing shortage that is happening at many hospitals.

Cheryl: You are very welcome.

Deanna: How do you view your role in transforming healthcare and making a difference in the lives of patients and families?

Cheryl: Prior to becoming the CNO of the hospital, I was the director of behavioral health at St. Joseph Mercy Ann Arbor. Working with patients in behavioral health is a life changing event. You are able to help change patients coping skills, it is extremely meaningful to help people and their families. Not just in behavioral health but in the entire arena of healthcare.  My role in transforming healthcare and making a difference is to collaborate, communicate and influence.

Deanna: What priorities are you looking at for transforming healthcare in the hospital?

Cheryl: Our Operating Priorities include:

  • Inpatient Medicine: Fully utilize acute care capacity and enhance access for communities served.
  • Inpatient Rehab: Expand inpatient rehab capacity in collaboration with Michigan Medicine.
  • Behavioral Health: Solidify optimal configuration to support safety and access.
  • Surgery: Achieve optimization of expanded surgery capacity.
  • Ambulatory: Grow primary and specialty providers to enhance access and provide the clinic space on campus.

We want to use what we have to full capacity to help those in the community and surrounding communities.

Cheryl: We are experiencing a nursing shortage and have notice we have drooped 2% from 2020 to 2021 for BSN prepared. We continue to be in the exceptional performance category (64% or greater) but do not want to continue to down slide. We have been looking at nurse turnover rates and why. Top three reasons that have been given are personal reasons, new job, and retirement. We then broke this down into categories that included, type of employee (regular, contingent, per diem), years of service, and age. We are now working on recruitment packages that are attractive and fair along with retention of existing employees.
