I was part of a healthcare team providing care to an 82-year-old AA male patient diagnosed with End-Stage Renal Disease (ESRD). The treatment options for the patient were long-term dialysis and a kidney transplant since his kidneys had permanently ceased functioning. The patient did not conform to the option of a kidney transplant since he perceived that it would be highly risky, and as a result, the team decided only to provide dialysis.

The team employed a client-centered approach by assessing and identifying interventions to meet the patient’s physical, emotional, and social needs. Furthermore, the patient was actively engaged in his care, and his preferences were incorporated in making treatment decisions. However, there was some power imbalance between the providers and the patient, as evidenced by some situations where the professionals tried to make the patient change his mind about not having a kidney transplant. The providers attempted to drive the patient towards accepting to have a transplant once a potential donor was identified.

The team was led by a clinical nurse leader (CNL) who facilitated effective communication among the team members. Communication was in the form of SBAR, a reliable and validated communication tool, which improved communication among providers, and enhanced patient safety (Erjavec et al., 2022). The CNL was tasked with coordinating the activities of each team member, including delegating tasks per a provider’s profession. Besides, the CNL ensured that there was no doubling of tasks and that the patient did not receive duplicated services (Shi et al., 2018). The team was comprehensive, with all professionals involved in the care of patients being represented. I was surprised that a social worker was part of the team, and this was crucial since he helped the patient obtain resources such as transportation and housing. The CNL assigned tasks and ensured that each member stayed on the task and implemented them promptly. The members decided the CNL should head the team since the scope of practice entails leading the healthcare team. The CNL maintained the leadership role throughout the patient’s hospitalization period.
