SIADH refers to an endocrine disorder typified by euvolemic hypotonic hyponatremia together with urine hyperosmolarity resulting from the enhanced release of antidiuretic hormone (ADH) from the pituitary gland, ectopic production of ADH, or genetic mutations that cause increased ADH receptor activation in the kidneys (Mentrasti et al., 2020). Hyperglycemia exceeding the renal threshold results in glucosuria which causes osmotic movement of water into the renal tubules resulting in polyuria. Subsequently, polyuria results in dehydration which stimulates the thirst center resulting in polydipsia (DiMeglio et al., 2018). However, polyphagia usually stems from the inability of glucose to enter the cells for utilization in energy production due to lack of insulin hence appetite stimulation and feeling of hunger. Genetic susceptibility and environmental triggers such as a prior viral infection stimulate an autoimmune response that results in autoantibody production i.e., anti-islet cell cytoplasmic antibody and anti-glutamic acid decarboxylase (DiMeglio et al., 2018). Autoantibodies significantly contribute to the progressive destruction of pancreatic islets resulting in absolute insulin deficiency.

Type 2 Diabetes

According to (Galicia-Garcia et al. (2020), the pathophysiology of type 2 diabetes is complex and multifactorial involving numerous genetic and environmental factors that ultimately result in peripheral insulin resistance (mostly due to central obesity and increased serine kinase activity) and pancreatic beta-cell dysfunction (due to accumulation of pro-amylin). Following a period of impaired glucose tolerance with postprandial hyperglycemia, overt type 2 diabetes manifests with fasting hyperglycemia.

Hypothyroidism

Hypothyroidism results from thyroid hormone deficiency. This may be caused by inadequate thyroid hormone production (primary hypothyroidism) in conditions such as Hashimoto’s thyroiditis, thyroidectomy, and iodine deficiency (Patil et al., 2022). Finally, pituitary disorders such as pituitary adenoma that cause TSH deficiency and hypothalamic disorders that result in TRH deficiency may cause secondary and tertiary hypothyroidism respectively.
