I will start by explaining what care coordination is. Care coordination is the activities or processes that aim to bridge gaps in patient care (NEJM Catalyst, 2018). Collaborating with patients and their families is fundamental to achieving health outcomes. One strategy that fosters patient collaboration is patient education. This strategy or patient experience is designed to help patients and their families improve health and prevent illness by expanding their knowledge and influencing their attitudes (Menear et al., 2019). As a nurse, you can provide patient education verbally or using brochures, manuals, books, videos, and websites.

The second strategy is adopting shared decision-making. This is a process by which health decisions are discussed and made collaboratively by the patient and the provider by considering the best available evidence, the provider’s clinical judgment, and the patient’s values and preferences (Menear et al., 2019). Another strategy is allowing patients to access their health records or patient portals. This allows them to take charge of their health and understand their progress with treatment.

Aspects of Change Management That Directly Affect Elements of the Patient Experience

Change management is an organized, structured approach with interventions that enable healthcare organizations to modify workflows smoothly. It is vital in enabling organizations to attain desired goals with minimal employee resistance to change (Harrison et al., 2021). Aspects of change management can impact patient experience if they entail changing practices that will improve the quality of patient care, patient safety, and costs. For instance, change initiatives that improve the quality of patient care bring a positive patient experience as they promote better health outcomes, reduce morbidities and mortalities, and decrease the length of hospitalization.

Furthermore, change elements that improve patient safety influence patients’ experience as they promote better outcomes and increase patient satisfaction with the care provided. In addition, aspects of change management that improve clinical processes and workflows significantly improve patient experience as they enhance patient care (Harrison et al., 2021). Improved workflows mean that patients do not have to wait long before being attended to in the ED, improving their experience and satisfaction with care.

Rationale for Coordinated Care Plans Based On Ethical Decision-Making

In our healthcare facility, we manage patients with complex healthcare needs. These patients are usually attended to by multiple specialists for different health conditions, including clinics and hospitals. Such patients would highly benefit from coordinated care plans. Coordinated care plans are essential in ensuring that all providers involved in a patient’s care are informed about the patient’s needs and preferences (Swan et al., 2019). They help to coordinate care and reduce wastage of resources and time. It is important to note that proper coordinated care plans should be concise, goal-oriented, and up-to-date.

When preparing coordinated care plans, the nurse should ensure that they are tailored to meet the patient’s needs, which upholds beneficence (the moral duty to do good). As the patient advocate, you ensure that the care provided meets the standards and does not risk patient safety to promote nonmaleficence. Moreover, autonomy should be upheld by engaging patients in decision-making and respecting their treatment preferences (Swan et al., 2019). Engaging patients when developing coordinated care plans helps them take charge of their health, which increases treatment adherence and promotes better health outcomes.

With a coordinated care plan, the nurse helps patients get the care they need while avoiding needless delays and duplication of care. Furthermore, coordinated care plans promote the ethical principle of beneficence by ensuring that all care needs for a patient, from medical care to long-term care planning and social services, are met in an organized and effective way (Swan et al., 2019). They also help to lower the incidence of gaps or duplication in patient care and offer increased flexibility when creating care plans to meet individual patient needs.

Potential Impact of Specific Health Care Policy Provisions on Outcomes and Patient Experiences

Healthcare policy provisions can impact patients’ health outcomes and experiences. The Affordable Care Act (ACA) is one policy whose provisions were linked with the potential to improve patients’ experience and outcomes. The ACA expanded insurance coverage, which was expected to increase Americans’ utilization of primary care and other healthcare services (Levine et al., 2022). Besides, the ACA has several approaches with the potential to control the increasing healthcare costs and lower medical errors, which improve overall health outcomes and patient experience.

The ACA also had a provision of expanding Medicaid eligibility to all persons under age 65 with an annual income of up to 133% of the federal poverty level to help low‐income persons afford healthcare insurance. This provision has the potential to expand insurance coverage and increase the affordability of insurance plans (Levine et al., 2022). Increased insurance coverage means more Americans can afford healthcare, improving overall health outcomes and patients’ experience with the healthcare system.

Another ACA provision enforces new regulations on insurance companies. For instance, insurers are prohibited from charging higher premiums or denying insurance coverage to persons with preexisting conditions. Furthermore, insurance policies must provide a minimum number of preventive services without cost‐sharing. This improves patients’ satisfaction since persons with preexisting chronic illnesses can afford health insurance and access medical services that improve their outcomes (Levine et al., 2022). Besides, individuals are covered to access essential healthcare services vital in improving health outcomes through preventive and curative services.

Nurses Role in the Coordination and Continuum of Care

In this session, I have discussed care coordination and the rationale for coordinated care plans. I will now discuss the nurse’s vital role in the coordination and continuum of patient care. It is crucial that we understand that nurses play a significant role in care coordination. Nurses take roles as case managers and patient navigators in care coordination. Karam et al. (2021) assert that case management is one of the most intensive interventions for caring for patients with complex health and social needs. Nurses engage in case management, which is a targeted, community-based, and proactive measure of care that entails case finding, assessment, care planning, and care coordination (Kelly et al., 2019). In addition, nurses have a role in patient navigation, linking patients and their families to primary care services, specialists, and community-based health and social services, which provide holistic patient-centered care.

The nurse’s role in care coordination further includes identifying patients who will benefit most from care coordination. The nurse collaborates with the patient’s PCP in identifying patients with complex health and social care needs and high healthcare costs (Karam et al., 2021). In addition, the nurse care coordinator examines comprehensive patient and family needs and goals, including caregivers’ burden.

The nurse is also tasked with creating individualized care plans with the patients and ensuring they align with their circumstances (Karam et al., 2021). The plan should provide the health and social care professional involved in the patient’s care with an overview of the patient’s health status and plans.

It is essential that we remember that care coordination seeks to promote appropriate and efficient delivery of healthcare services within and across healthcare systems. As nurses, we are responsible for designing coordinated care plans by ensuring patient care is provided on time and effectively. Besides, we must recognize that care coordination is a major responsibility for primary healthcare.
