The Ethical Application to Evidence
The reading for this week focuses on evidence-based practice (EBP) and the occasion when nurses do not use evidence to drive practice. We often are entrenched in the ways we have always done things. We are often convinced that we know something to be true because we were taught something years ago and have always believed it to be true. We do not dare to challenge our beliefs or practices for the fear that we have been delivering the wrong care or suboptimal care. This week’s reading gives a mere sample of practices that many of us currently employ and that have been proven wrong through evidence.
What is our ethical obligation to stay current in our practice? Nursing is a profession that demands lifelong learning to consistently deliver quality care. Practice changes as new evidence or research is brought to light. This week, as you search for ethical evidence to tie to your topic, reflect on your ethical obligation to use EBP to drive your practice. Consider whether there is liability attached to any nursing practice that is not grounded in evidence. This question will pave the way for next week’s topic on legal issues in nursing.

Many nurses believe that their employer’s malpractice insurance will protect them in the event of a patient-care error. Most nurses do not realize that the malpractice insurance will only protect them if they followed their employer’s policy and procedure. Consider the number of times each day that you observe a coworker “working around” a policy. This is a prime example where the facilities’ malpractice insurance would not cover an individual. All nurses (regardless of their role) are encouraged to carry their own malpractice insurance. It is your professional responsibility.
As a future Doctor of Nursing Practice (DNP), you are tasked with modeling appropriate behavior for all nurses. All nurses should be aware of their responsibilities in providing safe care. If you are currently a nursing leader, delve deep to find out why your nurses are doing a “work-around.” Your role in healthcare as a future DNP is to lead change and develop practices and policies where a “work-around” is not needed to deliver safe care.

Clinical Seminar
In your clinical seminar this week, you will present a professional 12- to 15-slide Microsoft PowerPoint presentation (exclusive of references) to your classmates. The presentation will be a collation of the evidence (articles) you have retrieved about your topic of interest. Although you will use the articles that you have retrieved each week as the basis for your discussion question, the evidence will be presented in one culminating project in support of nursing standards and patient safety toward your topic of interest.
While critiquing your weekly articles, you may find similarities or distinct differences within the evidence. Explore the reasons for this. Take note of the strength of the evidence reviewed. In your Microsoft PowerPoint presentation, present and analyze the evidence and how it supports nursing standards and patient safety. Your classmates will have an opportunity to ask thought-provoking questions about the evidence you present.

What does the evidence say about your topic?
According to Stevens (2013):
The impact of evidence-based practice (EBP) has echoed across nursing practice, education, and science. The call for evidence-based quality improvement and healthcare transformation underscores the need for redesigning care that is effective, safe, and efficient. In line with multiple direction-setting recommendations from national experts, nurses have responded to launch initiatives that maximize the valuable contributions that nurses have made, can make, and will make, to fully deliver on the promise of EBP. Such initiatives include practice adoption; education and curricular realignment; model and theory development; scientific engagement in the new fields of research; and development of a national research network to study improvement.
To affect better patient outcomes, new knowledge must be transformed into clinically useful forms, effectively implemented across the entire care team within a systems context, and measured in terms of meaningful impact on performance and health outcomes. The EBP movement began with the characterization of the problem—the unacceptable gap between what we know and what we do in the care of patients. (para. 1–2)
Given the catalyst for the EBP movement several years ago, we know that gaps still remain despite our best efforts. What gaps still exist today pertaining to your topic of interest? The retrieval of topic-specific evidence over the last several weeks must have revealed some disparities in healthcare delivery. Please stop and reflect on these areas and be sure to include your findings in your Clinical Seminar presentation this week.
