Technology and Telehealth

The National Patient Safety Goals is a program by The Joint Commission aimed at enhancing overall safety of patients and ensuring that they get better outcomes. The home care goals under this program recognize the threats that exist in care delivery and the need for nurses as primary care providers to ensure that they implement the provisions for quality care delivery. The Joint Commission reviews these goals each year and for 2022, the first goal is improving the accuracy of patient identification. The goal implores nurses to use two identifiers like the name and date of birth. The rationale for this goal is to reduce wrong patient error and increase the ability to reliably identify the individual. Secondly, the goal ensures that the right individual gets the correct treatment or service (The Joint Commission, 2022). The second goal is to improve the safety of using medications. Patients in home care or hospice care may not understand their medication needs and should not be expected to understand the interactions among them. Therefore, nurses have a duty to help them understand by knowing the medications that one is taking. The implication is that medication reconciliation is essential to avoid any potential errors. The third goal in infection prevention where nurses are expected to help in reducing the risk of getting healthcare associated infections. This implies having proper hygiene etiquette and practices that include hand hygiene, respiratory hygiene, and environmental hygiene as well as effective use of personal protective equipment (PPEs) to reduce the risk for infections among the vulnerable patients in home care situations.

It implies that nurses should identify any risks in home care settings and provide necessary awareness and education to attain the expected outcomes. Nurses can use signs in the setting as a reminder of oxygen storage to enhance patients’ awareness and prevent any adverse events.   

Technology-based interventions are improving the quality of care and interactions between providers and patients in home care settings. An article by Stome et al. (2020) discusses the use of web-based communication platform to enhance home care services in Norway. The authors assert that the integration of web-based technology in home care will improve communication and involvement of patients, lower overall costs and expenses, and increase efficiency. Using a pilot study of a technology-based application, DigiHelse in home care settings in major municipalities in Norway, the authors’ findings demonstrate that technology can improve overall care delivery through effective communication. These include information for family members that plan to make visits, and notifications of such visits for better planning and integration of other components of care delivery. The article is emphatic that improved communication will help nurses and other providers in these settings to interact well with the patients and develop effective approaches to delivery of quality care. This shift to home care will influence the interventions that nurses use to communicate with patients and increase their involvement. The implication is that more patients will get quality care in these settings because of better understanding between them and providers.
