Therefore, it is important for healthcare professionals to use appropriate means and strategies to ensure that such individuals have the needed patient care. Based on the condition, some of these patients usually experience various difficulties such as access to food, which can be caused by factors such as poor socioeconomic background, living in a food desert, lack of an appropriate transportation system, or a combination of two or more factors (Hawkins& Panzera, 2021). As part of an effective approach, healthcare professionals such as nurses can come up with meal plans that can be used to help such patients have opportunities to gain adequate access to appropriate and affordable foods. Therefore, the purpose of this assignment is to select an individual from my chosen family who has one or more chronic health issues and create a meal plan for the individual.

            It is important to consider the patient’s health conditions when coming up with a meal plan. The patient has Ankylosing spondylitis, which is known to cause inflammation. Shopping Locations, Food banks, Meal storage and Meal Preparation

            There are various shopping locations, including Townmall Westminster, Westminster Shopping Center, 140 Village Shopping Center, Carroll Plaza Shopping Centers, and Westminster Station. There are also various food banks in the area, including human services programs of Carroll County, Community Media Center food drive, Carrol County Food Sunday, and Westminster Seventh-day Adventist church community services (“Carrol County,” n.d). Meal storage is mainly done using freezers to avoid spoiling. As part of the plan, the food will be prepared with the help of family members such as the boyfriend.

Transportation Means, Routes, and Timing

The Carrol Area Transit System (CATS) is the main public transportation system. This system serves the area and surrounding areas within the county. The system operates various fixed routes. The system operated from Monday to Friday from 7 am to 5 pm. However, earlier or later services are usually available. Costs vary from $1 to $ 10, whether it is a demand response or Trailblazer, while children under the age of five are usually given free rides. The buses have several routes that they ply throughout the day (“Carrol County, 2021). Even though the boyfriend owns a car, the patient has no care, has no driving license, and fears to drive. Therefore, public transportation will mainly be used as the boyfriend also goes to work.

Support Services

            There are various support services that can be of help to the patients, including the public transport system (CATS), meal assistance services as the patient also struggles financially, and healthcare services since she can use the hospitals and clinics in the area. The patient can also use mental health services for counseling (“Carrol County,” n.d). Another support service that the patient may require is a community support system.

The Individual’s Medical Conditions and Cultural Preference

            Health conditions and cultural preferences usually determine health plans. The individual’s health or medical condition is Ankylosing spondylitis. The patient experiences pain due to the condition and feels that her condition will stop her from working before she is forty. The patient identifies with the Latino culture.

Available community Services and Referrals

            There are available community services which can be beneficial to the patient. The community services include human service programs, the county health department, which offers various services, and the Department of Social Services. The patient is struggling financially and may not be in a position to buy food appropriately. Therefore, one of the referrals is that the patient needs to get in contact with the supplemental nutrition assistance program (SNAP) in the region (“Carrol County,” n.d).

Interprofessional Nature of Working With Families

            It was important to engage with the family throughout the process and thereafter. A phone call as a way of follow-up showed that the meal plan worked for them since they were also provided with the locations. They also managed to largely stay within the budget. An interprofessional nature of working with the families was evident when they had to contact a nutritionist who gave appropriate pieces of advice regarding finding alternative foods with the same nutritional value as the ones that the family could not get from the store.

Conclusion

            This assignment has focused on a meal plan for a patient with Ankylosing spondylitis. The plan entails using a balanced and healthy diet for a duration of one week. In addition, various aspects such as shopping locations, food banks, and support services have all been explored.
