2. Community Health Needs Assessment

Select a zip code near you to use as the target community in this reflection and later assessments. Select the zip code that includes the city government offices (city hall, city library) as well as retail stores, schools, and housing.

Review at least 1 Community Health Needs Assessment (CHNA) report as prepared by the closest nonprofit hospital serving the selected community.

Health Indicators

            The HealthyPeople 2030 is a national database that provides national objectives that should be met to improve the health and wellbeing of the population over the next decade. The database provides information about 358 core, measurable, developmental, and research objectives that are crucial for the promotion of population health. Among the data contained in the database are leading health indicators. Leading health indicators are the subsets of high priority areas in HealthPeople 2030 objectives. The subsets guide the small, systematic strategies that are adopted to improve the health of the population. Most of the leading health indicators address the population’s impact of major causes of morbidity and mortality in the United States, Organizations, states, and communities use the indicators to prioritize the interventions adopted to address health disparities(health.gov, n.d.). The selection of leading health indicators is based on the crucial health issues with immense impact on the population.

            Global health issues determine health indicators. For example, disparities in access to healthcare services delay the diagnosis and treatment of health problems such as hypertension and heart disease. Issues such as the high cost of healthcare services limit the access to high-quality healthcare services to address the prioritized health needs of a population. Lifestyles and behaviors such as sedentary behaviors among the population also contribute to the worsening burden of diseases such as obesity and diabetes. The leading health indicator topic, which is most important for my practice setting is obesity. The number of patients diagnosed with obesity and its associated conditions such as hypertension and diabetes has been rising significantly. Factors such as unhealthy dietary habits and sedentary behaviors may be contributing to their development(Mensah et al., 2019). Therefore, population-centered interventions are needed to address the problem.

Community Health Needs Assessment

            The HealthyPeople 2030 explores several health issues of crucial importance to public health. One of the presented health issues is overweight and obesity. The data presented in the website shows that two in every five adults and one in every five children and adolescents in America have obesity. Many others in the population are obese. Obesity predisposes the population to a range of health problems such as type 2 diabetes mellitus, cancer, hypertension, stroke, and heart disease. Individuals from some ethnic/racial groups are increasingly predisposed to obesity as compared to others(health.gov, n.d.). Public health nurses can support the various levels of prevention for obesity and overweight. They can adopt interventions such as health education, screening, early treatment, and rehabilitation of the populations at risk and affected by overweight and obesity, and their associated comorbidities.

            The community of focus is XXX. The health issue of concern is diabetes. Primary, secondary, and tertiary level prevention interventions can be applied to diabetes mellitus. The primary level interventions that can be adopted include health education and screening. Health education raises awareness among the population and promotes lifestyle and behavioral change. Secondary interventions include the initiation of early and timely treatments for those found with diabetes. Early treatment prevents the risk of patients developing complications and improve the prognosis of diabetes treatment. An example of a tertiary prevention intervention is rehabilitation. Rehabilitation entails providing care services that promote patient’ coping with diabetes (Correia et al., 2019; Sun et al., 2022). It also aims at minimizing the risk of diabetes-related complications and prolonging life.
