After working with my preceptor to assess the organizational policies, I have created various measurable outcomes from this EBP project. The proposed nursing intervention in this project is the adoption of hourly rounding to manage and prevent patients’ falls in nursing homes by attending to the patients’ needs proactively. Hourly rounds connote a practice of nurses making planned visits to the patient wards and conducting particular nursing interventions on an hourly basis (Pellicier Mercucci, 2020). The interventions include providing nourishment, pain management, and ensuring pertinent musculoskeletal needs such as proper positioning, range of motion, and ambulation are realized. The intervention also involves clearing walking areas of clutter and ensuring medical equipment is effectively functioning. Below is the list of outcomes for the proposed nursing intervention.

Reduction in Chances of Falling
The adoption of hourly rounds is projected to manage and prevent falls by supporting patients using various interventions including promoting education about falls and fall prevention to create awareness about fall management. The other intervention is the utilization of various techniques such as the adoption of technology and the eradication of environmental hazards. These interventions are expected to reduce the chances of falling by improving patients’ independence, health, and quality of life (Pellicier Mercucci, 2020).
Reduction in Risk of Injury
The successful implementation of the proposed nursing intervention is expected to assist nursing homes to manage and prevent falls by removing potential environmental hazards, adopting technologies such as bedside bells to notify clinicians about patient falls, and educating care providers about fall prevention and the safety of patients. Implementation of the proposed nursing intervention will also help healthcare providers to develop effective policies and techniques to manage and prevent falls in hospitals (Montero-Odasso et al., 2021).
Maintaining Mobility
The fundamental objective of fall prevention is to enhance or maintain mobility and encourage independence. The implementation of the proposed nursing intervention is projected to encourage interventions such as regular exercise and activities that foster mobility and independence such as walking, workouts, and jogging (Montero-Odasso et al., 2021). These activities are critical in reducing the risk of falling and maintaining mobility by enhancing strength, coordination, balance, and flexibility.
Improvement in the Quality of Life
The adoption of the proposed nursing intervention is projected to yield positive patient outcomes in the management and prevention of falls including increased satisfaction with healthcare, reduction in healthcare costs, and improved psychological and physical comforts. These factors cumulatively improve quality of life (Stevens & Lee, 2018).
Prevention of Costs Associated with Patient Falls
Patient falls are usually linked to severe injuries and are among the most expensive healthcare conditions among patients in nursing homes (Stevens & Lee, 2018). However, the implementation of the proposed nursing intervention is projected to eradicate costs associated with patient falls such as increased cost of care, patient mortality, increased injuries, and longer hospital stay days. Conclusion
The adoption of hourly rounding to manage and prevent patients’ falls in nursing homes by attending to the patients’ needs proactively is projected to yield positive outcomes. Some of the outcomes include such as a reduction in the chances of falling, reduction of the risk of fall-related injuries, maintaining patient mobility, improvement in the quality of life, and prevention of costs associated with patient falls. The intervention is expected to improve care outcomes.
