Practice Problem

Offer strong evidence-based statements for the significance data, including prevalence, and/or incidence, and/or mortality data at the national and state (or county or city) levels. If available, include data from the practicum site to support this is a priority problem for the organization.

Include the project aim (main purpose or objective) at the end of this section.

Include in-text citations.

Use bullet points for practice problem data points. Bullet points are not required for project aim.

Use concise and accurate statements.

Practice Question

The practice question must be the exact approved PICOT practice question as identified in the Chamberlain University IRB approval letter.

Remove the name or identifiers of the practicum site from the practice question and other sections of the poster.

Methodology (Part 1 and Part 2 in the Rubric) (Students conducting the case study should leave this blank.)

In this section, students who are implementing at the practicum site are required to state the information in short concise phrases with bullet points. Use the exact subheaders listed on the NR 707 poster template.

State the translational science model or theory/change model.

Provide a short description of the setting.

Provide a short description of the population with the estimated number of participants.

Briefly state the inclusion and exclusion criteria.

Briefly describe the intervention (cite primary source)

Briefly describe the formative evaluation plan.

Briefly describe the summative evaluation plan.

State the main outcome in the PICOT practice question.

Briefly describe the data collection (e.g., pre-intervention and post-intervention data collection).
