Efficacy of Mindfulness-Based Stress Reduction in Alleviating Anxiety Levels among Adult Psychiatric Patients: A Intervention Study.

The DNP Project focuses on an evidence-based intervention aimed at ameliorating healthcare outcomes by addressing the escalating anxiety levels prevalent among adult psychiatric patients. Anxiety disorders, affecting millions worldwide (Yang et al., 2021), significantly impact healthcare domains such as quality, safety, effectiveness, patient-centeredness, timeliness, cost-effectiveness, and equity (Kelley et al., 2022). These disorders contribute to a more intricate and costly treatment process, often intertwined with coexisting conditions. Moreover, anxiety plays a pivotal role in healthcare inequality, hindering individuals’ access to healthcare services and exacerbating disparities in utilization and outcomes.

To address this clinical problem, the DNP Project introduces a Mindfulness-Based Stress Reduction (MBSR) intervention. This evidence-based approach aligns with current research and offers a promising avenue for mitigating anxiety in psychiatric patients. By integrating mindfulness practices, the intervention aims to enhance the overall well-being of individuals grappling with anxiety, fostering improvements across multiple healthcare outcome domains. The holistic nature of MBSR provides a comprehensive strategy to address the complexities associated with anxiety disorders.

This DNP manuscript unfolds in distinct sections, each contributing to a thorough understanding of the intervention’s development, implementation, and evaluation. The subsequent sections will delve into the project’s organizational setting, population details, the theoretical framework guiding the intervention, project management, sustainability plans, and the comprehensive data collection and analysis strategy. As readers navigate through these sections, they will gain insights into the systematic approach employed to tackle the identified clinical problem and the potential impact of the MBSR intervention on healthcare outcomes.

Problem

Anxiety disorders, including generalized anxiety disorder, panic disorder, and social anxiety disorder, pose substantial challenges due to the significant distress and functional impairment they induce. The pervasive impact of these disorders transcends age cohorts, cultural backgrounds, and socioeconomic statuses, creating a widespread healthcare concern nationally and globally. In 2019, the global prevalence of anxiety disorders reached approximately 301.39 million cases, with 45.82 million new cases reported (World Health Organization, 2022). This places an immense burden on individuals and imposes a considerable strain on healthcare organizations.

The consequences of anxiety disorders extend beyond individual suffering, contributing to 28.68 million disability-adjusted life years in 2019 (Yang et al., 2021). The financial implications associated with anxiety disorders are noteworthy, involving both direct costs related to medical and mental health services and indirect costs arising from diminished productivity and economic strain on families and communities. While the global economic burden is significant, anxiety disorders also profoundly impact the United States, where they stand as one of the most prevalent mental health conditions.

Despite the evident societal and economic costs, there is a notable gap in addressing anxiety disorders comprehensively within healthcare systems. This project aims to fill this gap by introducing an evidence-based intervention, the Mindfulness-Based Stress Reduction program, to alleviate anxiety among adult psychiatric patients. By doing so, the project seeks to not only enhance individual well-being but also contribute to the overall resilience and effectiveness of healthcare organizations in managing and mitigating the impact of anxiety disorders.

The presence of anxiety disorders has implications for the quality of healthcare delivered by organizations. Patients with anxiety face complex healthcare requirements that may prove difficult to address within conventional care models. Anxiety can worsen physical health conditions, hinder treatment adherence, and result in frequent emergency department visits and hospital readmissions (Garakani et al., 2020). Consequently, healthcare organizations must navigate the challenge of managing both the mental and physical aspects of patient care, emphasizing the need for effective interventions.

The community-level challenges faced by the practicum site, mirror those experienced on a broader scale. Anxiety significantly influences both patient well-being and healthcare utilization. While specific statistics on anxiety prevalence within Desai Medical Group’s psychiatric patient population are not available at this point, anecdotal evidence suggests a concerning pattern. Many patients grapple with chronic and severe anxiety, leading to prolonged treatment periods, heightened reliance on medication, and, in some cases, necessitating readmission. This not only strains healthcare resources excessively but also poses potential risks to patient well-being. Recognizing this, there is an imperative to explore evidence-based non-pharmacological interventions that seamlessly integrate with the existing treatment approach. This project, through the implementation of the Mindfulness-Based Stress Reduction program, aims to address this urgent need and contribute to more effective and holistic anxiety management within the Desai Medical Group community.

At the practicum site, the predominant approach for managing anxiety in psychiatric patients is centered around pharmacological interventions. While these interventions demonstrate a certain level of effectiveness, they are accompanied by potential adverse effects and may not adequately address the comprehensive needs of patients. Moreover, it is widely recognized that certain anti-anxiety medications have acknowledged limitations and can cause adverse effects. The practicum site acknowledges the necessity for alternative or additional therapies to enhance patient care. In their pursuit of comprehensive and potentially side-effect-free approaches, they are actively exploring evidence-based interventions to address anxiety symptoms. By introducing a mindfulness-based stress reduction program, we can bridge this gap in practice and fulfil our mission of promoting the overall well-being of psychiatric patients.

Project Aim and Supporting Objectives

Aim

To assess the effectiveness of a mindfulness-based stress reduction program in alleviating anxiety among adult psychiatric patients at The practicum site over an 8-10-week timeframe.

Objectives

  1. To implement a program at The practicum site that focuses on reducing stress through mindfulness techniques, taking into account the specific requirements of each patient.
  2. To evaluate the levels of anxiety in participants before and following an intervention lasting 8-10 weeks.
  3. To provide training and education for nursing and therapeutic staff members on the principles and approaches of mindfulness practices to ensure program consistency and quality throughout its duration.

Practice Question

The following practice question will serve as the basis for the DNP project: For adult psychiatric patients, does the implementation of a mindfulness-based stress reduction program, compared to current practice, impact anxiety levels in 8-10 weeks?

Research Synthesis and Evidence-Based Intervention (NR 702)

Evidence-Based Intervention

State the name and brief description of the evidence-based intervention and identify the national organization or government agency that endorses the intervention, if applicable.

Evidence Synthesis

Start the synthesis section telling readers this is an evidence synthesis of the research supporting the intervention (state the intervention and cite the research articles). State the number of articles in the synthesis that support the intervention (10 peer-reviewed research articles published in the last 5 years are required). In a brief summary statement, provide the numbers of research articles that are level I, II, and III and give a summary of the quality of the research articles (cite the research articles for each category).

Main Themes in the Research

[An example of themes would be if the researchers discuss an important aspect of the intervention in the research results for diet and exercise in reducing obesity in age children. In the results or discussion section of the articles, the researchers talk about school and family support being integral to the success of the intervention. Some other research articles about the same intervention might mention culturally accepted foods boosted the success of diet behavior changes. These are two themes in this example.]

Name of First Theme to Discuss. Begin this paragraph right after the and discuss the first theme here and cite the articles where this theme is found. State how this teme is related to

the success of the intervention.

Name of Second Theme to Discuss. Begin this paragraph right after the and discuss the second theme here and cite the articles where this theme is found. State how this teme is related

to the success of the intervention.

Name of Third Theme to Discuss.Begin this paragraph right after the and discuss the third theme here and cite the articles where this theme is found. State how this teme is related to the success of the intervention.

Contrasting Elements in the Research

Explain the differences in the research articles. Contrasting points in the research can be different inclusion and exclusion criteria, different populations, different study timeframes, and different aspects or elements considered or controlled.

Research Support for the Evidence-Based Intervention

Present an objective summary statement supporting the evidence-based intervention. In this section, summarize your research support for your intervention. Summarize the number of articles and levels of evidence in which the research results demonstrate the intervention is effective in changing your outcome and/or the practice problem. Think of this section as similar to the closing arguments in a trial. In the closing arguments, the lawyers summarize the main points they want the jurors to remember. In this section, discuss the strong and compelling

evidence supporting your intervention!

Methodology (NR 702)

Please write an introductory statement that includes the parts of this section that will be reviewed. Refer to the assignment guidelines and rubric for the exact content required in

this section.

Organizational Setting

Translation Science (or QI Model + Nursing Theory) and Project Management

Theoretical Framework:

Refer to the assignment guidelines and rubric for the exact content required in this section. According to the APA manual section 4.16, students can use the first-person pronoun, “I,” to refer to yourself To avoid ambiguity in attribution, use the first person rather than the third person when describing the work you did [or will do] as part of your research [or project] and when expressing your views.” Identify the theory or model that will serve as the framework for the project; translational science models are recommended. Identify the primary concepts or steps of the model.

Project Implementation Plan

Create the steps in your DNP practice change project that coincide with the theoretical concepts. Create your project implementation plan that includes the timeline of weeks for the project (usual projects are 8-12 weeks, keeping in mind the requirement to implement the intervention for at least eight weeks). Use the research articles to guide your intervention implementation plan. Create a detailed description of each step in the intervention implementation. Describe in detail how each participant will engage with the intervention from the beginning to the end of the intervention. Discuss who will implement the intervention, who will receive the intervention, when, where, and how. Describe in detail how each participant will engage with the intervention from beginning to the end of the intervention. The evidencebased intervention implementation is the foundation for your project and will drive improved

outcomes. Please be specific here and be clear on the details.

The intervention implementation must be a full eight weeks while the project implementation might be 8 to 12 weeks.Include the plan for the pre-and post- summative data collection. Include the weekly plan for the formative evaluation to ensure high intervention fidelity. Develop a week-to-week formative evaluation plan utilizing observation, 1:1 discussions, huddles, leadership meetings, or other processes to describe your project management oversight during implementation.

Required Resources and Proposed Budget (NR 705)

Refer to the assignment guidelines and rubric for the exact content required in this section. Explain the resources needed for successful project implementation (conference room, supplies, project information, handouts, etc.). Refer your reader(s) to your budget table in the Appendices and ensure that the budget table is completed and is balanced. If there is an expense, there must be an associated revenue source. For example, if staff training is required, the staff training hours will be paid by the practicum site. The proposed budget for the project should

be balanced.

Results (NR 709)

Refer to the assignment guidelines and rubric for the exact content required in this section. Present the results of your project. Represent the data from statistical results in tables or figures. Explain the results and what the results mean in the context of the purpose of the project and your stated outcome in your practice question.

Conclusions (NR 709)

Refer to the assignment guidelines and rubric for the exact content required in this section. Conclusions indicate what is known regarding nursing practice when your results and results from prior literature are considered together. Conclusions should relate directly to your purpose and practice question. You can discuss your thoughts on what may have affected the results. If your results are similar to those found in previous studies, you may state that. However, if your results are completely different and/or contradict previous studies, you should

let the reader know that these results cannot be used beyond the project population and setting.

Clinical Relevance (NR 709)

Refer to the assignment guidelines and rubric for the exact content required to be in this section. Conclusions  should relate directly to your purpose and project question. They are generalizations that loop back to the existing literature on your topic. For each conclusion you make, cite the sources that support or contradict your findings. The conclusion should represent the contribution your practice project has made to the body of scientific knowledge on this topic and relate this to the significance of the project, which is always, in some way, to improve nursing practice. Conclusions indicate what is now known regarding nursing practice when your results and results from prior literature are considered together.

Implications for nursing or clinical relevance should answer: What do the findings mean to nurse leaders, and would society care about the results? Recommendations based on the findings should be for the nursing profession and to specific nursing leaders. Be sure to make specific recommendations for leaders in the nursing field and/or policy makers.
