Much of an archeologist’s work is done under the mantra “proceed with caution.” Archeologists must dutifully secure permissions to access sites. They also must exercise extreme caution when excavating or analyzing in a lab to avoid potential damage to historical artifacts.
Likewise, nurse practitioners must proceed with caution when building a patient’s health history. Important questions can be difficult for both nurse and patient. Care must be taken to approach such questions with dignity, tact, and respect to create an environment conducive to productive conversations.
More importantly in today’s society, the possible Social Determinants of Health for each of our patient’s also needs to be taken into consideration

Review the media programs related to a vaginal exam, pap test, and breast exam.
Review the health history guide presented in Chapter 7 of the Schuiling & Likis (2022) text and consider how you would create your own script for building a complete health history. (Note: You will also find the Health History Form in Chapter 7)
Provide all the components of a complete gynecologic health history. Include considerations for special populations such as LGBTQ+ individuals.

Develop your own script for building a complete health history and as you create your script, consider the difficult questions you want to include in your script. There is no sample template to provide to you. (Utilize chapter 7 of your Schuiling textbook to provide guidance). You are the one to develop the script. Think of it as you are writing a movie and you need to write the script for the movie. What lines would you provide for the actor to utilize when sitting down with a patient to perform a COMPLETE Medical History which also entails those DIFFICULT GYN questions. You do not need to provide the answers to the questions however, if you find that beneficial, you may do so.
Assignment: (1- to 2-page reflection)
In addition to your script for building a health history for this assignment, include a separate section called “Reflection” that includes the following:
A brief summary of your experiences in developing and implementing your script during your health history.
Explanations of what you might find difficult when asking these questions. What you found insightful and what would you say or do differently.

HPI: Susan Lang is a 24-year-old Caucasian female presenting to the clinic for regular care. Susan relates she has been having some postcoital bleeding for the past 6 weeks and has had a sore throat for the past 3 weeks. She did have a fever for a day or two, but Tylenol took care of it and she thought it was allergies.
CAGE Questionnaire:
She has not felt the need to cut down on her drinking, feeling that “everyone drinks like she does.” She is occasionally awakened in the am sleeping next to a stranger but does not remember how it happened. She has been occasionally annoyed by her parent’s criticism about drinking but has been better since she moved into her apartment. She relates she does not feel guilty about drinking. She has never taken an “eye opener.”
Current Medications: OTC Pamprin for cramps during menstruation
Allergies: NKDA

Soc & Substance Hx: works full-time as an administrative assistant, cigarette smoking at a rate of ½ packs per day (PPD) since age 14, / EtOH only on weekends, 6-8 hard liquor/ daily, and marijuana smoking

Pamprin. Susan relates she “sometimes” uses condoms but feels like she gets irritated by them. She does not desire pregnancy but is not sure what type of other options are available. She relates she has had 13 partners in the last 12 months, her partners have all been male except one in the past year. She has never received the Gardasil vaccine as her mother doesn’t believe it’s been tested enough and reports no history of STIs but doesn’t remember being tested in the past.
ROS:
GENERAL: No weight loss, fever, chills, weakness, or fatigue.
HEENT: Eyes: No visual loss, blurred vision, double vision, or yellow sclerae. Ears, Nose, Throat: No hearing loss, sneezing, congestion, runny nose, or sore throat.
SKIN: No rash or itching.
CARDIOVASCULAR: No chest pain, chest pressure, or chest discomfort. No palpitations or edema.
RESPIRATORY: No shortness of breath, cough, or sputum.
GASTROINTESTINAL: No anorexia, nausea, vomiting, or diarrhea. No abdominal pain or blood.
NEUROLOGICAL: No headache, dizziness, syncope, paralysis, ataxia, numbness, or tingling in the extremities. No change in bowel or bladder control.

Encounter for gynecological examination (general) (routine) with abnormal findings-
With all women’s well exams, it is included to do a routine gynecological exam with a potential pap smear when indicated. For women under 21 who are not sexually active, the pap smear would not be indicated, those over 21 but under 30 will only need this exam every 3 years, and those over 30 with no abnormal results can wait every five years for the exam. For this patient, she is over 21 and sexually active with no known pap smear so one would be indicated for this visit. She is also at high risk for HPV due to her multiple partners and not receiving the vaccine. (Mishra et al.,2023.)
Encounter for Screening of Infections with a Predominately Sexual Mode of Transmission-
Screenings for sexually transmitted infections are important with every well-women exam. Understanding which patients are at risk and what preventative measures are needed is vital to any visit. The CDC has guidelines that help any provider decide which tests are needed and when they need to be screened. It is also important to provide all types of screening which would include blood testing, vaginal swabs, and urine samples to fully exclude all types of infections. (Territo & Burstein, 2023.) This patient has been with multiple partners with little to no forms of birth control and has some symptoms of an infection therefore it is imperative to test.
Gonococcal infection, unspecified-
Gonorrhea is a sexually transmitted infection caused by the bacterium Neisseria gonorrhoeae. Infection may involve the genitals, mouth, or rectum. Infected men may experience pain or burning with urination, discharge from the penis, or testicular pain. Infected women may experience burning with urination, vaginal discharge, vaginal bleeding between periods, or pelvic pain. Many of those infected, however, have no symptoms. (Van Ommen et al., 2023.) The patient does have symptoms that would match this infection and she did test positive for gonorrhea.
Trichomonas vulvovaginitis-
Trichomonas is caused by infection with the parasite Trichomonas vaginalis, which is transmitted almost exclusively through vaginal sexual intercourse. Common symptoms can include an unusual discharge from the penis or vagina, itching of the genitals, a burning sensation when urinating, and pain during sexual intercourse. (Kaplan, 2023.) The patient is at high risk for this due to her increased number of sexual partners and did test positive.
Chlamydial infection, unspecified-
Chlamydia is a sexually transmitted infection caused by the bacterium Chlamydia trachomatis. Most people who are infected have no symptoms. When symptoms do appear, they may occur only several weeks after infection; the incubation period between exposure and being able to infect others is thought to be on the order of two to six weeks. Symptoms in women may include vaginal discharge or burning with urination. (Van Ommen et al., 2023.) The patient does exhibit some of these symptoms and with testing was found positive for Chlamydia.

Reflection
When reviewing this case there were a couple of questions missing that would provide more information for the visit. The patient did not give much of a history of any violence or safety. Considering she has stated that she is sexually active with multiple partners it would be important to ensure she is safe and feels safe in her relationships. When it comes to testing, I would add syphilis to the testing as it has had an increase in the last couple of years. I would also have a conversation about birth control forms, and possibly discuss a long-term birth control option.
Regarding the patient, plenty of health promotion and disease prevention can be done. Depending on her living conditions, she would need to be educated on fire safety, carbon monoxide safety, and safe living conditions. She has no medical conditions or diseases that warrant education on prevention. It is also important to discuss safe sex practices with each visit considering she is sexually active, with no real means of birth control, and has had multiple partners.
It is also important with a patient who has multiple positive infections to allow dignity. As providers, our job is to treat the patient and allow them to feel they are in a safe and non-judgmental environment. Therefore, it is important to remain objective and leave any prejudices outside the clinic. If these patients are judged or feel they are being judged for their actions they are less likely to seek treatment or continue treatment.

· CV: regular sinus rhythms without murmur or gallop
· Abd: soft, non-tender, liver normal,
· Breasts: fibrocystic changes bilaterally, no masses, dimpling, redness or discharge, no adenopathy, and bilateral nipple piercings.
· VVBSU: wnl, slight frothy yellow discharge by cervix, clitoral piercing noted
· Cervix: friable, some petechia no cervical motion tenderness.
· Uterus: mid mobile, non-tender
· Adnexa: without masses or tenderness
· Perineum: wnl
· Rectum: wnl
· Extremities: full rom, skin clear, no edema, reflexes 1+.
· Neurological: CN II-12 grossly intact.
Diagnostic results:
Pap returns as normal, with trichomonads noted.
Throat culture returns positive for gonorrhea.
Vaginal culture returns positive for chlamydia.
Wet mount shows many WBCs, many trichomonads, positive clue cells, and positive amine (whiff test).
RPR negative, hepatitis c negative, HIV negative

Trichomonas is an STD caused by infection with Trichomonas vaginalis parasite (Centers for Disease Control and Prevention [CDC], 2022a). Symptoms vary from person to person and may include Itching, burning, redness, soreness, dyspareunia; and a clear, white, yellowish, or greenish vaginal discharge with a fishy smell. Without treatment, the infection can last for months or even years (CDC, 2022a). Patients should be advised to not have sex until they and their sex partners are treated. Trichomonas has a high rate of reinfection, retesting is recommended three months after initial treatment for all sexually active women (CDC, 2022a). Patients who have tested positive for trichomonas should also be tested for syphilis (CDC, 2022a).
Chlamydia most frequently reported bacterial infectious disease in the United States (Centers for Disease Control and Prevention [CDC], 2022b) Most people are asymptomatic however some early symptoms may include vaginal discharge and pain with urination (CDC, 2022b). Treatment includes Doxycycline 100 mg PO 2 times a day for 7 days; patients should be advised to not have sex until they and their sex partners are treated. Because of the high rate of reinfection, retesting is recommended three months after initial treatment (CDC, 2022a).
Gonorrhea is a sexually transmitted disease that infects the mucous membranes of the reproductive tract; it can also infect the mucous membranes of the eyes, mouth, throat, and rectum (Centers for Disease Control and Prevention [CDC], 2023b). Many infections are asymptomatic; early symptoms may include dysuria, increased vaginal discharge, and vaginal bleeding between periods (CDC, 2023b). Treatment includes a single dose of Ceftriaxone 500 mg IM. Test-of-cure is needed 7-14 days after treatment for people who are treated for pharyngeal (infection of the throat) gonorrhea (CDC, 2023b).
Plan

Urine pregnancy test.
Syphilis testing.
Therapeutic interventions: Metronidazole 500 mg 2 times/day for 7 days.
Education: Do not have sex again until you and your sex partners are treated (when therapy has been completed and any symptoms have resolved). The best way to prevent genital trichomoniasis is through consistent and correct use of condoms (external or internal).
Follow-up visits: Retest for trichomonas in three months NAAT.
Educated the patient on the need to use barrier protection to minimize risks of reoccurring STI.
Initiate HPV series.
Educate the patient on contraceptives.
Order contraceptives based on patient preference.
Chlamydia
Therapeutic interventions: Doxycycline 100 mg orally 2 times/day for 7 days.
Education: Complete all antibiotics. Do not have sex again until you and your sex partners are treated (when therapy has been completed and any symptoms have resolved).
Follow-up visits: Retest for chlamydia in three months.
Gonorrhea
Therapeutic interventions: A single 500 mg intramuscular dose of ceftriaxone.
Education: Wait one-week post injection before having intercourse.
Follow-up visits: A test-of-cure is needed 7-14 days after treatment for people who are treated for pharyngeal (infection of the throat) gonorrhea. Retested three months after treatment of the initial infection.
