INTRODUCTION

As a registered nurse or APRN, no doubt the issue of managing patient pain is one you have dealt with repeatedly. In identifying successful approaches to addressing and alleviating pain, you may have looked to theories of pain management to inform your practice.
The work you begin this week will guide and prompt you in the further application of theory to nursing practice. You will start with an overview of middle range theories, which in the world of nursing theory refers to theories that relate to particular phenomena, such as pain. Your study of nursing theory will be made more meaningful as you identify a practice issue of special interest to you and investigate nursing theories that apply to it.
As you continue to develop your philosophy of nursing practice, consider middle range nursing theories that may be defining for you. Expanding your familiarity with theories that inform the many dimensions of nursing will support your leadership as a DNP.

This practice issue can also be one focus of your Module 3 exploration of evidence-based practice and quality improvement, and your Module 4 investigation of a critical practice question. INVESTIGATING A CRITICAL PRACTICE QUESTION THROUGH A LITERATURE REVIEW

This week you will begin your Module 4 Assignment, which is intended to inform and support your planning for your DNP Project in your program of study. This Assignment has two parts: a literature review in the Walden Library of research to address the critical practice question that you framed in Week 7, and a critical assessment of your search outcomes in which you synthesize the evidence. Both Parts 1 and 2 will be due by Day 7 of Week 10. Plan your time accordingly.

WEEKLY RESOURCES

To prepare:

  • Analyze your critical practice question from the Week 7 Discussion. Based on feedback from colleagues and your Instructor, revise your question, as necessary, to ensure a clear and relevant focus for your literature search.
  • Review the Learning Resources to support your search.
  • Access the Individual Evidence Summary and Synthesis Tool document from the Learning Resources. This is a word document that you will complete with at least 10 scholarly articles relevant to your critical practice question. Scan the template to ensure you understand how to enter information for each article you identify.
  • Use the Week 8 Discussion to guide and clarify your search process and to pose any questions related to the template categories, search terms, or others.

The Assignment

Part 1: Literature Review (10+ scholarly articles)

Using the Walden Library as your source, search to select at least 10 scholarly articles that represent current literature (i.e., published within the previous 5 years) with evidence that addresses your critical question and could inform a practice change initiative for quality improvement. Using the Individual Evidence Summary Tool/Evidence Synthesis Tool template document, complete all sections for each article.

Part 2: Critical Assessment (7+ pages)

In a paper of at least 7 pages, plus cover page and references page, include the following:

  • Write a critical assessment of your search outcomes that synthesizes the evidence from your literature review.
  • Demonstrate clear connections between the practice problem that informs your critical question, your appraisal of evidence that addresses the critical question, and resulting clarification on the need for a practice change initiative focusing on quality improvement. Be specific and provide examples.

NURS 8114 WEEK 11 DISCUSSION: TRANSLATION OF EVIDENCE AND APPLICATION

Based on your work in previous modules, you will wrap up the course with a Discussion on challenges in translating and applying evidence to implement a quality improvement initiative. Draw on your experience in your current or previous health care settings to consider specific barriers to address and opportunities to leverage in advocating for evidence-based practice quality improvement.

  • Review the readings in the White, Dudley-Brown, and Terhaar text. With your current health care organization, or an organization you are targeting for your DNP Project, in mind, consider the area(s) of greatest challenge with regard to translating and applying evidence for a practice change initiative, e.g., leadership, technology, collaboration, stakeholder buy-in. Focus on the relevant text chapter(s) in your Discussion preparation.
  • Consider theories and best practice recommendations for addressing your identified challenges and barriers to translating and applying evidence that would support practice change initiatives.
  • Reflect on the philosophy of nursing practice that you developed in Modules 1–2. Consider your role as a DNP in creating an organizational culture that embraces evidence-based practice and quality improvement.

With these thoughts in mind …

BY DAY 3 OF WEEK 11

Post an explanation of the challenges and barriers to translating and applying evidence for practice change in your target health care organization. Briefly explain your issue(s) of concern and describe specific approaches for addressing these challenges. Explain how you view your role as a DNP in creating a health care culture that promotes translation of evidence for quality improvement and explain why. Then, recommend actions and activities you could model and lead, including through an EBP QI project, to advocate for quality improvement and social change in nursing. Be specific and provide examples.
